- Conduct initial assessments with clients to identify their individual barriers to healthy food access.
- Design nutritional service plans to meet the needs of each client, this may include:
- provision of food vouchers,
- procurement of household kitchen supplies,
- enrollment in Project Bread cooking classes,
- nutrition counseling with a Project Bread dietitian for individuals with eligible diagnoses, and
- connections to SNAP, WIC, School Meals, and other foods assistance programs.
- Maintain connections with clients to ensure that the designed service plan is meeting their needs and adjust as appropriate.
- Conduct follow up calls with each client at three- and six-months post enrollment
- Maintain accurate records in Salesforce.
- Ongoing communication with health center staff to ensure optimal support for each member.
- Promote usage of Project Bread's FoodSource Hotline.
- Perform other related tasks as needed.
- Haitian Creole Language Responsibilities:
- Conduct initial assessments and follow up calls in Haitian Creole
- Ongoing phone conversations, texts and emails to provide technical assistance
- Interpret cooking classes
- Review translated materials for cultural and contextual relevance
- Participate in planning and evaluating the annual Walk for Hunger, including working as event staff the weekend of the Walk (first weekend in May), and attending pre-Walk events as assigned.
- Participate in organization-wide events and meetings.
- Act to promote, sustain, and grow Project Bread's positive, productive, and inclusive working environment.
- 2+ years of experience providing direct services to low-income individuals.
- Background in nutrition, client services, and/or health care.
- Database experience (Salesforce preferred).
- Strong written and oral communication skills.
- Attention to detail.
- Ability to manage multiple tasks simultaneously.
- Desire to be a part of a strong and supportive team.
- Experience working in diverse environments with people of different backgrounds.

Supervisor: Assistant Director Health Care Partnerships Program
Salary Range: $60,000-$62,000
The Role: The Nutrition Services Coordinator is to respond to referrals and develop service plans customized to meet the individual needs of each client so that they can access the healthy foods needed to best manage their health conditions. The Coordinator provides ongoing case management support for each client for their six-month program enrollment. This position will primarily work with members remotely, but the role may include some in-person visits on an as needed basis.
About Flexible Services
The Commonwealth of Massachusetts received a waiver from the Centers for Medicaid and Medicare to improve the ways in which MassHealth provides care to its members. This waiver led to the establishment of a pilot Flexible Services Program. Through the Flexible Services Program, Accountable Care Organizations (ACOs) may pay for health-related nutrition and housing supports as part of their strategy to address social determinants of health.
Project Bread is an approved nutrition service provider for six ACOs. In this role we receive referrals and provide nutritional services to members from over 30 health centers, hospitals, and physician offices across Massachusetts.
