Resident Service Coordinator

The Resident Service Coordinator is located in a senior housing complex. You will be providing information, referral, service coordination, activities planning, and follow-up to all building residents.

Your caseload will include residents of the building, but also members of the Revere community that are consumers of Mystic Valley Elder Services.


Schedule:
This is a Local Union 509, full-time, non-exempt (35 hour/week) position. We offer 4-day work week and hybrid work schedule (once training is complete.)
No nights or weekends.

Compensation:

Starting annual salary is $51,038.08 Being bilingual is not a requirement for this role, but we recognize it as an asset and offer an additional 6% bilingual designation for Spanish, Haitian Creole, Russian, Vietnamese, Cantonese or Mandarin.

What you'll be Responsible for:

Essential functions of the position are below. Additional duties may be assigned as required.

  • Conduct initial and ongoing assessment of consumers' health and functional status.
  • Conduct visits as required in various settings including consumers' homes, community settings, and/or medical facilities.
  • Participate in development of consumers' care plans, including determining appropriateness and need for inhome/communitybased services.
  • Arrange, coordinate, and oversee inhome and communitybased services for consumers.
  • Maintain consumer records in accordance with standards.
  • Compile and submit required reports.
  • Coordinate activities and presentations for residents.
  • Collaborate with management of the housing company/owner.
  • Collaborate with contracted provider agencies to coordinate and assign service staff to consumers/residents.

Qualifications:


  • A Bachelor's degree in Social Work or a related field.
  • Social Work license preferred.
  • Knowledge of and experience working with older adults in a communitybased setting preferred.
  • Strong interviewing and assessment skills.
  • Strong interpersonal and organizational skills.
  • Excellent written and verbal communication skills.
  • Computer skills: Proficient with MS Office Suite and entering narrative and other data into a database. Ability to use multiple databases. Ability to use the Internet to conduct information searches
  • Private transportation.
