Summary of Care Management Coordinator (CMC):WIHD is recruiting for a dedicated and compassionate Care Management Coordinator.
This unique position includes on-site responsibilities, allowing you to make a meaningful impact on individuals with intellectual and developmental disabilities.
They will coordinate and advocate care between the patient, medical team, and the medical neighborhood in order to enhance access, improve quality, and increase practice efficiency and effectiveness.
The ideal candidate will have a license or registration in nursing or as a clinical medical assistant.The CMC position is valuable to WIHD in that they represent the outpatient clinic in a positive and professional manner and be able to handle difficult situations calmly and refer to other staff when necessary.
This position is full time and located at our Valhalla clinic location. The CMC is eligible for medical, dental, vision, paid time off, Term Life, STD, LTD and 403(b) Match.

WIHD addresses major social and health issues by developing and delivering medical, clinical and support services to individuals, their families and caregivers.
Major Responsibilities:
Chronic Care Management:
Regularly confirm and ensure the appropriateness of scheduled appointments, tests, procedures, and forms for patients enrolled in the CCM program.
Provides follow-up management with patients to ensure compliance with their individual care plan
Ensure accurate and comprehensive documentation of patient's CCM note in the electronic health record.
Communication Management:
Attend and participate in all required meetings and contribute to effectively collaboration within the healthcare team. Answer telephone calls, emails, patient portal messages, tasks, faxes, and health information exchange notifications.
Handle routine information requests or collaborate with appropriate staff for resolution.
Provide assistance for completing requests such as referrals, medication refills, prior authorizations, transportation requests, and pre-certifications.
Education, Knowledge, and Skill Requirements:
Education, Licensure, Certification:
Associate Degree Preferred
Certification in a related health field such as Social Work, Nursing or Certified Clinical Medical Assistant
Experience and knowledge:
3+ years of administrative experience
2 years in a health care setting with applicable/relevant experience
MS Office Suite; EMR knowledge preferred
Experience working with individuals with disabilities (Especially I/DD) is Preferred
Essential leadership, advocacy, communication, and research skills
Understands and follows confidentiality rules and regulations
