Shift:
Full Time, Days, 9:00 a.m. - 5:00 p.m.

In this role, you will coordinate and participate in activities related to care management services to family members and caregivers.

Utilize patient-centered motivational interviewing techniques to build rapport and help patients improve their health.

Use team-based communication strategies to close the loop on referrals, hospital follow-ups and any outstanding items identified in the patient's care plan.

Perform outreach activities in primary care sites, homes, hospitals, and neighborhoods.

Job Qualifications:_
High school diploma or equivalent required.
One to three (13) years of relevant experience, required.
Bachelor's or master's degree in relevant field strongly preferred.
Health home or care management experience strongly preferred.
NYS Driver's License strongly preferred.
Various positions available in NYC/Bronx, Staten Island and Brooklyn.

The salary range for this position is $44,450-$69,340/year.
