Job Summary:
We are seeking a dedicated and compassionate Care Coordinator to join our team. As a Care Coordinator, you will be responsible for managing and coordinating services for individuals with developmental disabilities.

Your role will involve relationship management, program development, and administrative tasks to ensure the highest level of care and support for our clients.


Requirements include:


  • Active NY Registered Nurse license
  • 23 years experience in health care related discipline and/or discharge planning or community healthcare experience
  • BSN preferred
  • Excellent interpersonal skills
  • Ability to respond to short deadlines

Pay:
$ $68.10 per hour
