    Position Summary: The Family Program Coordinator (FPC) partners with families (adult women and their infants/children) at the Pico Home as well as our Alumnae Program, where they transition to independent living. The FPC coordinates services and events for our alumnae including case management, home visits, leadership opportunities via the Alumnae Leadership Council (ALC), monthly and quarterly community events such as Diapers & Donuts and the end of summer party, ongoing training and workshops, and resource linkage. The FPC also provides on-site case management services to our two family program participants at the Pico Home.

    Weekly Schedule / Availability

    Full-time, 40 hours per week with flexibility and availability in hours and days to work nights and/or weekends, as needed. One weekend shift per month required for Alumnae-based activities. Must be willing to travel around LA County for home visits.

    Responsibilities:
    Case Management:

    • Provide case management for the Family Program at Pico Home and the alumnae of Harvest Home's residential shelter program, implementing the Critical Time Intervention model for those within their first-year post-graduation from Harvest Home, which includes home visits.
    • Collaborate with the Care Coordination's interdisciplinary team regarding client transitions and needs.
    • Meet with clients one-on-one to establish goals and action plans which facilitate a successful transition and stabile living plan.
    • Initiate follow ups, assist in overcoming barriers, guide decision-making, and empower clients in self-advocacy.
    • Facilitate linkage to mental health, substance abuse, medical services, and other community resources while providing transportation when needed.
    • Develop and maintain close working relationships with various service providers, outreach workers, case managers, and collaborative agencies.
    • Submit requests for financial assistance on behalf of clients with partner organization.
    • Interface and attend meetings as directed with collaborative agencies and other community meetings as directed.
    • Engage with external organizations to keep abreast of new, changing, and/or additional opportunities to ensure case management services and resources are appropriate, relevant, up-to-date, and applied to client and program needs.
    • Input, maintain, and organize detailed, accurate, high-quality documentation in resident files, database, and logs.

    Alumnae Programming and Events:

    • Oversee and coordinate quarterly community and holiday events for 25-50 alumnae and their children, and current residents, including the Mother's Day event, the End of Summer BBQ, and Christmas party.
    • Facilitate alumnae connections through Harvest Home's alumnae Facebook group and other social media outlets.
    • Coordinate monthly Saturday Diapers & Donuts (diaper and wipes pick-ups) for alumnae. Provide drop-offs and pick-up alternatives if available.
    • Coordinate Alumnae Leadership Council (ALC) including facilitating quarterly meetings and strategic planning for alumnae involvement. Ensure ALC maintains minimum # of participants (6-8 alumnae actively involved).
    • Provide ongoing workshops and other training opportunities for alumnae based on assessed needs and ALC feedback, such as financial literacy, housing retention, child development, etc.

    Staff Duties:

    • Attend staff meetings, trainings, and events.
    • Meet with supervisor to discuss case plans, progress with alumnae, challenging issues, etc.
    • Other duties as assigned.

    Approach

    • Build rapport and respect with clients as a foundation for professional relationships. Partner with clients providing support, guidance, encouragement and accountability as they work to achieve personal and program goals.
    • Utilize a variety of approaches as appropriate for care, guidance, intervention, and/or service delivery such as Trauma-Informed Care, Resident-Centered, Solution-Focused, and an empowering strengths-based approach.
    • Listen attentively to client concerns and suggestions and address their issues fairly and professionally, coordinate with immediate supervisor for supervision.
    • Model Harvest Home's cultural values in communication, behavior, and professionalism.

    Qualifications and Skills

    • Ability to work well both independently and collaboratively as a member of a multi-disciplinary service team.
    • High level of self-accountability and ethical standards.
    • Resourceful, motivated self-starter with the ability to manage time well and work autonomously; to manage multiple tasks and priorities, including proficiency in balancing direct service responsibilities with program, documentation, and organization responsibilities.
    • Excellent critical thinking skills, solution focused approaches, and crisis intervention skills with ability to exercise and model good judgment.
    • Excellent relational and interpersonal communication skills, employing active listening skills, and motivational interviewing techniques.
    • Proven ability to communicate with diverse audiences including clients, staff, volunteers, community members, resource providers, and other stakeholders.
    • Strong administrative skills and proficiency in Microsoft Office Suite (Word, Excel, Outlook) and online client database required.
    • Ability to set, model, and uphold appropriate personal and professional boundaries and relationships with staff, clients, volunteers, and community providers.
    • Ability to work with a small, close-knit staff team and uphold a teachable spirit that is open to receiving feedback from others.
    • Act as a role model to implement and uphold organizational values.

    Education and Experience

    • BSW; or other equivalent education or commeasurable experience.
    • 1+ years case management experience with a similar population in a relevant setting.
    • Professional experience with developing, planning, and executing a variety of internal and external events. This includes identifying and working with a variety of external venues/facilities, negotiating contracts with vendors, and managing event budgets.
    • Experience working with many of the following: high-risk populations, women, homelessness, pregnancy, infancy, victims of trauma, mental health, substance use, domestic violence, poverty, TAY, Foster care, DCFS, etc. in social welfare, social work, counseling, hospitality, and related fields Experience working with diverse, high-risk populations.
    • Preference given to candidates with human services experience in LA County and with knowledge of local agencies and organizations.
    • Preference given to candidates with knowledge of or experience with infant/child development.
