- Conduct thorough patient assessments to determine eligibility for home infusion therapy, considering medical condition, living situation, and potential barriers
- Collaborate with the billing department to verify insurance coverage related to home infusion therapy
- Coordinate with healthcare providers to transition patients from clinic to home infusion therapy
- Educate patients and families about the home infusion process and medication delivery
- Maintain accurate electronic records and monitor patient progress
- Ensure compliance with healthcare regulations, standards, and organizational policies
- Build strong relationships with pharmacy and clinical teams to facilitate medication ordering and delivery
- Efficiently manage patient refill schedules to ensure continuous care
- Knowledge of home infusion therapy, including TPN therapy and administration methods
- Strong communication skills to engage effectively with healthcare professionals and patients
- Exceptional attention to detail and organizational skills to handle multiple patient cases
- Proficiency in electronic medical record systems and relevant software applications
- Ability to work independently and collaboratively with a problem-solving mindset
- Experience in sterile compounding preferred
- Certified pharmacy technician license strongly preferred
- Minimum 1 year of experience in a Home Infusion pharmacy, specifically in the intake department working with TPN
- In-Person role located in Orange, CA
- Monday-Friday schedule from 8:00am to 4:30pm
