    Job Summary

    As the Home Infusion Intake Coordinator at AmeriPharma's Specialty Care IV division, you play a crucial role in ensuring seamless and top-notch home infusion therapy services for patients. Your responsibilities include working closely with healthcare professionals, patients, and insurance providers to facilitate the intake process for home infusion therapy, requiring dedication to patient care and strong coordination skills to ensure a smooth transition.

    Duties and Responsibilities

    • Conduct thorough patient assessments to determine eligibility for home infusion therapy, considering medical condition, living situation, and potential barriers
    • Collaborate with the billing department to verify insurance coverage related to home infusion therapy
    • Coordinate with healthcare providers to transition patients from clinic to home infusion therapy
    • Educate patients and families about the home infusion process and medication delivery
    • Maintain accurate electronic records and monitor patient progress
    • Ensure compliance with healthcare regulations, standards, and organizational policies
    • Build strong relationships with pharmacy and clinical teams to facilitate medication ordering and delivery
    • Efficiently manage patient refill schedules to ensure continuous care

    Required Qualifications

    • Knowledge of home infusion therapy, including TPN therapy and administration methods
    • Strong communication skills to engage effectively with healthcare professionals and patients
    • Exceptional attention to detail and organizational skills to handle multiple patient cases
    • Proficiency in electronic medical record systems and relevant software applications
    • Ability to work independently and collaboratively with a problem-solving mindset
    • Experience in sterile compounding preferred

    Education and Experience Requirements

    • Certified pharmacy technician license strongly preferred
    • Minimum 1 year of experience in a Home Infusion pharmacy, specifically in the intake department working with TPN

    Schedule Details

    • In-Person role located in Orange, CA
    • Monday-Friday schedule from 8:00am to 4:30pm

    Physical Requirements
