    The SPF-P4S program is grounded in the Strategic Prevention Framework is designed using a community engagement model grounded in public health principles, including being data-driven, and focused on providing evidence-based services to high-risk underserved communities.

    It is the responsibility of the Substance Abuse Prevention Project Manager (SAPPM) to identify local substance use prevention concerns and use data to identify prevention priorities in the community, develop, and implement strategies to prevent the misuse of substances and promote mental health amongst youth and adults.

    This position is responsible for the development of community and related stakeholder relationships and develop a community coalition drawing from public health, community organizations, treatment facilities, school districts, law enforcement, etc) to address prevention, treatment and recovery services to advance substance abuse prevention efforts across the community. The SAPPM is responsible for leading the Cedar Branch Community Advisory Group to attain the requirements of the grant.


    The SAPPM is responsible for the day-to-day activities of and community outreach for the SPF-P4S Grant and the administrative supervision of the grant.

    This includes the supervision of Aspf-p4s Epidemiologist.

    The selected candidate will work closely with other supporting departments to ensure timely and accurate data and grant reporting detailing the progress towards grant deliverables.

    The SAPPM must ensure that grant efforts and data collection are all using evidence-based practices per SPF-P4S guidelines leading to full completion of deliverable goals for the SPF-P4S grant.

    Must be able to demonstrate knowledge and skills necessary to perform all job-related activities as outlined below.


    Essential Duties and Responsibilities:

    Primary Functions:
    Provide daily oversight of the grant.
    Oversee the implementation of the project activities, internal and external coordination, development of materials, and facilitation of meetings.

    Develop a community infrastructure to address prevention, treatment, and recovery services; utilize community coalition building strategies to advance substance abuse prevention efforts across the community.

    Develop a highly functional coalition utilizing the Strategic Prevention Framework guiding principles that include assessment, planning, implementation and evaluation models to effectively address the substance abuse and related mental health problems facing the communities of the service are.

    Lead and assist the project team in the completion of a qualitative and quantitative needs assessment, ensuring use of qualified state, county and local data.

    Lead and support the development and Implementation Plan that includes evidence-based strategies.

    Provide leadership for daily operations including, but not limited to, training and development, providing budgetary information, providing data and research, attending/convening/facilitating partner meetings, and monitoring of deliverables.

    Develop project activities, including securing locations, assigning staff, ensuring materials are available, etc.
    Plan and facilitate all Cedar Branch Meetings ensuring the alignment with the grant.
    Monitor all grant contracts/agreements.
    Coordinate/provide training to community and behavioral health professionals.

    Work collaboratively with SPF-P4S staff, other SDAIHC staff, and community partners to build meaningful collaborations that help address substance use presentation, treatment and recovery services.

    Document and track data that demonstrates efforts to meet grant deliverables.
    Analyze data and adjust efforts as needed to ensure grant deliverables are being met.
    Provide educational presentations to community groups and funding agencies as needed.

    Work collaboratively with SDAIHC Grant writers and Data team to ensure monthly, quarterly and yearly reports are completed and turned in on time.

    Attends SPF-P4S grantee meetings.


    Qualifications:

    Minimum

    Qualifications:

    Master's degree (Ph.
    D. preferred) in public health, communications, substance abuse treatment/prevention or a related field.
    Proven track records of organization, collaboration, and outreach skills.
    Minimum 3-year experience working in Grant funded programing.

    Preferred:
    Experience serving a multicultural population.
    FQHC background.
    Familiarity with Community Health Clinics and/or Indian Health Clinics.
    Experience working in the substance abuse prevention field.
    Bilingual English/Spanish.

    Special Conditions of Employment:
    Current BLS and CPR certification.
    Has a clean driving record and insurance as required by the state.
    Has reliable transportation.
    Monday - Friday, 8:00am to 5:00pm Schedule, on-site
