    Job DescriptionSalary: $34,850.00-$40,000.00

    SUMMARY OF POSITION:

    The Clinic Care Coordinator (CC) will assist clients who are experiencing or have recently experienced homelessness with accessing medical care and local resources. The CC is responsible for coordinating medical referrals and appointments, linking clients to benefits and external referrals, coordinating transportation as needed, and assisting individuals with self-management of chronic diseases. The CC is expected to understand homelessness, mental illness, and addiction; and be willing and ready to engage with clients who struggle with these issues at all levels. The CC will report to the Lead Clinic Coordinator.

    RESPONSIBILITIES:

    · Provide services at one or more of our clinic sites: (PSH) Permanent Supportive Housing Sites, Reed Clinic, Caroline Clinic, or Cathedral Clinic.

    · Assist clients in linking to medical care and other community based social services, including referrals from other agencies.

    · Assist clients with benefit applications, such as SNAP, TANF, Medicaid, Healthy Texas Women, Metrolift, and disability.

    · Assist clients with accessing services offered through their medical benefits (Medicaid, Medicare) or county hospital services.

    · Follow-up with clients regarding appointments, care plans, and health goals.

    · Coordinate transportation for clients to/from medical appointments, possibly including accompaniment as needed.

    · Document interventions, plans, follow up, and plan of care in Electronic Medical Records and other data systems, as assigned

    · Provides information, referrals, advocacy, outreach, and prevention education services, as needed, targeting individuals in need of medical care and/or with chronic disease.

    · Provides culturally and linguistically appropriate services to clients.

    · Completes accurately, and in a timely manner, all documentation and reports.

    · Develop relationships with area social service agencies to build knowledge of the resources available to clients.

    · Participate actively in scheduled meetings, training sessions, conferences, seminars and independent study.

    · Works toward developing a broad knowledge of casework and carries the volume of work as determined with his or her supervisor.

    · Candidate will have or obtain the following certifications during the first year of hire: Community Partner program navigator, SOAR certification, and Community Health Worker certification.

    · Will learn and use various computer programs, including EPIC (electronic medical record), HMIS, TMHP, and Novitasphere.

    · It is the responsibility of every employee to report any patient safety concern to their immediate supervisor without the fear of reprisal.

    · Occasional travel for meetings and client appointments.

    · Other duties may be assigned.

    SKILLS:

    Strong organizational skills.

    Strong interpersonal and social skills with demonstrated ability to collaborate with a variety of individuals from a wide range of professional and personal backgrounds.

    Good oral and written communication skills.

    Able to operate small office equipment, including photocopier, scanner, telephone, and personal computer.

    Basic knowledge of Microsoft Office.

    Ability to navigate local transportation services (i.e. bus, train, and taxi).

    PHYSICAL REQUIREMENTS:

    · Lift up to 25 pounds (laptop computer, small office equipment use, or similar items)

    · Must be able to stand, sit, walk, kneel, and bend.

    QUALIFICATIONS:

    High school diploma or GED

    Certified Community Health Worker preferred.

    Ability to be trained to perform a variety of tasks.

    Bilingual English and Spanish preferred

    Candidates with personal experience of homelessness are strongly encouraged to apply.

    PERSONAL CHARACTERISTICS:

    Ability to convey a strong presence, professional image, and deal confidently with complex situations.

    Maintain a professional disposition while working with a multidisciplinary health care team.

    Enthusiasm, interest, and ability to empathize with those who are homeless.

    Fosters a respectful sensitivity and non-judgmental attitude towards clients that may have a history of homelessness, mental health concerns, health concerns, substance abuse, and/or multiple disabilities.

    Committed to addressing problems and issues constructively to find mutually acceptable and practical solutions.
