Who we are looking for:

We are looking for a
licensed CNA with
at least 4 years of hands-on, verifiable CNA experience, and at least 1 years of healthcare-related supervisory experience. Above-average care skills, leadership, attention to detail, and compassion are key qualities required by the position.

As our Client Care Coordinator, you will train staff, oversee clients, work shifts, and cover shifts as needed.


Summary of Job Description:


As our Client Care Coordinator, you will be the liaison between staff, family members, other Agency Managers, and healthcare professionals (on occasion).

The most important responsibilities of this position are to make sure that all client care is covered, health and safety regulations are met and Agency policies are being adhered to by staff.

You must ensure all shifts are covered, plans of care for clients are adhered to, and facilitate regular communication with all staff regarding directives, plans of care and policies.

In addition, the Client Care Coordinator will coordinate with the team to ensure clients are comfortable and cared for, and important and key issues are communicated to the families, other staff, and the Clinical Care Manager.


Must be available 7AM - 8PM; including some holidays and weekends.

The Client Care Coordinator performs and is responsible for the following tasks:

Ensure all client care is covered
:


  • Take an active role in the care and ADL's.
  • Ensure daily shift changes are performed and/or following up with shift change documentation.
  • Monitor care to ensure accuracy and thoroughness by day/evening staff.
    Approves care daily through EMR system. Agency tablet will be issued.
  • Ensure the best care is provided to our clients by all staff members enforce health and safety regulations and that all requested care services are provided to our clients by staff.
  • Oversee that all policies, procedures, and protocols are adhered to, including communicating all changes and new information to all staff.
  • Assess care plans every 30 days (or more often, if necessary) and reporting to your direct supervisor any changes that may be needed for care.
  • Commitment to ensure clients maintain dignity and independence for as long as possible (age in place)
  • Provide instruction, training, and information to staff, as appropriate; ensure they are knowledgeable of their duties, rights, and responsibilities.
  • Promote good communication among staff, families, Supervisors, and our company Management by means of meetings, written, and verbal reporting. Encourage openness at all levels.
  • Monitor staff duties and performance. Develops and administers evaluations.
  • Commitment to promoting and developing the highest quality of care standards for older adults.
  • Monitor care via EMR System and random site visits (sites can include care/living facilities and client homes)

Pay:
From $17.00 per hour
