General Statement of Duties:
Care Managers conduct assessments, develop care plans and provide ongoing monitoring within an atmosphere of person centered planning with clients.
Specific Duties/Responsibilities:
Be the primary contact with the client and the client's family unless other arrangements are specified in the plan of care.
Cooperate with the client's legal representatives or other individuals for which consent has been given by the client or client's representative.
Provide client advocacy, crisis intervention, and referral services to the client and the client's family.Provide program information that explains the options under the programs and answers client questions.
Direct efforts to maximize the potential of the informal support system and encourage better community independent living capability.
Conduct initial assessments, reassessments, reevaluations and status reviews that adhere to the principles of person centered approach to care plan development and negotiated risk.
Assist the client with the completion and submitting of any required forms included but not limited to DSS' CHCP Special Eligibility Determination document.
Conduct care management activities only after the completion of the initial comprehensive assessment and development of the plan of care.
Authorize the start of service delivery for enrolled service providers.Ensure the timely discontinuance of services when appropriate.
Collaborate with and involve all providers that serve a particular client at all points of the care management process.
Coordinate the delivery of all services in the plan of care regardless of the provider or source of reimbursement, to avoid duplication and overlapping of services to monitor service quality and quantity and maintain the informal network.
Develop working relationships with nursing facilities and/or hospitals to develop policies and procedures in order to access necessary information such as hospital or facility records as allowed under federal regulations.
Document care management in the plan of care and all CHCP activities in the client's record.Provide care management only to people who are not living in an institutional setting such as a hospital or nursing facility unless they are institutionalized for respite care.
Ensure that community based services are not continued during a period of institutionalization unless transition services are subsequently authorized.Ensure that care management is not provided to people living in an institutional setting unless they are there for respite care.
Provide information and service referral or access to appropriate resources on a 24 hour per day basis, including responding to emergencies.
Work collaboratively with the DSS PSE unit to report suspected abuse, neglect, exploitation and/or abandonment of CHCP participants.Adhere to requirements set forth in DSS' CHCP Guidelines for Coordination Between the Protective Services for the Elderly Program, the CHCPE, Alternate Care Unit, Contracted Access Agencies and the courts.
Special note that applies to all CHCP job descriptions: It is the intent of WCAAA to review in a fair and thorough manner, all grievances and appeals filed and/or noticed to WCAAA or DSS and to cooperate with DSS on any such notices at a minimum by:Attending hearings at the request of DSS and represent the DSS when necessary
Document all grievances filed and their outcomes
Assist the Department in the preparation of summaries for Fair Hearings when an appeal is made to DSS including a client reevaluation upon DSS requested.
QUALIFICATIONS:
RN degree with current active CT. license or social worker who is a graduate with a Bachelor's degree of a four year college or university.
Minimum of two years of experience in health care or human services at the client care level.
A Bachelor's degree in nursing, health, social work, gerontology or related field may be substituted for one year of experience.
Licensure or Board certification preferredThrough work and/or educational experience, demonstrated interviewing skills which include the professional judgment to probe as necessary to uncover underlying concerns of the applicant; experience in the conduct of social and/or health assessments, knowledge of human behavior, family/caregiver dynamics and a range of disabilities.
Demonstrated familiarity with community resources in the state & 41 towns as well as ability to understand and apply a variety of complex eligibility & reimbursement dynamics to evaluate, negotiate and plan for various care options.
Through work and/or educational experience, ability to establish and maintain compassionate and supportive relationships and demonstrate skills in person centered approach to care plan development.
Car and valid driver's license required; travel throughout the 41 towns is required.Familiarity with computers required as well as capacity to carry a laptop.

General Statement of Duties:
Full time position aimed at risk mitigation and prevention of institutionalization for seniors and younger disabled individuals through care management services.
Daily tasks include conducting comprehensive in‐home assessments of seniors or younger disabled individuals to identify strengths and areas of need.
Care Plan and goal development and implementation of services will help support an individual to remain safely in their homes.
A care manager will provide ongoing monitoring, advocacy and support within an atmosphere of person centered planning. This position acts as the primary service contact with the senior or younger person with a disability (or families).Care Managers may be required to help the client/family with required paperwork to secure and maintain benefits including but not limited to Medicaid, Medicare, housing, and transportation.
Participation in various interdisciplinary meetings concerning the individual's wellbeing and any status changes is necessary and may require travel beyond the WCAAA region.
Remote work is available based upon completion of training curriculum and demonstrated skill proficiency after 6 months of employment.Qualifications :
Bachelor's or Master's degree in Social Work or a related field. LMSW or LCSW preferred.
Bachelor's degree in Nursing with current active CT license; two years of demonstrated related experience working in health and human services.
Acquired Brain Injury Care Managers must possess an LMSW, LCSW, or a Master's degree in Human Services, Counseling or Rehabilitation Counseling, in addition to two years demonstrated experience.
Vehicle and valid CT driver's license required as this position requires travel. Organizational skills, excellent time management, professional interpersonal skills, and knowledge of community resources. Assessing individuals in their homes, a nursing or other skilled facilities will be required as needed. Second language is a plus.
