The Care Coordination Manager manages the total scope of centralized follow-up and care navigation performed at PPSNE, as well as direct supervision to associated staff.

Coordinates the daily operations of the Care Coordination department under the supervision of the Medical Director.

This position works in conjunction with the Directors of Clinical Services and Clinical Quality Improvement to ensure follow-up is done according to PPFA Medical Standards and Guidelines and facilitates patient access to recommended care.

Functions as a liaison with the call center, health centers, administrative staff, and external partnerships including, but not limited to CT Department of Public Health, RI Department of Public Health, Quest lab, Early Detection Program and other care navigation partners, to ensure that all programs and functions are consistent with the mission and core values of the agency and advance PPSNE's strategic objectives to maximize mission impact.


Key responsibilities and accountabilities include:

development and implementation of systems and processes that increase quality of care, assessment and communication of program needs and outcomes to PPSNE leadership, and continuous evaluation to ensure these programs are meeting patient needs and organizational goals.


Essential Functions:

Supervises all members of the Care Coordination team, including care navigators, clinic assistants, licensed clinicians, and nurses

Supports staff in implementing agency priorities and customer requirements

  • Expands the team's perspective by communicating a clear vision of the "bigger picture" including the organization's strategic initiatives and the ways in which the care coordination program advances those objectives and increases mission impact.
  • Maintains ongoing and continual feedback with staff members to identify and remove barriers to success and create effective and efficient systems with staff input
  • Assures that obstacles to customer service within the staff's sphere of influence get prioritized and addressed, and the obstacles outside their sphere of influence are brought to the appropriate forum.
  • Supports crossprogram processes for identification and assignment of targeted populations to maximize effectiveness of care coordination program.

Demonstrate ongoing proficiency in electronic practice management systems and electronic health records

Creates and updates Care Coordination policies and procedures for PPSNE and oversees their implementation in conjunction with the Director of Clinical Services and Director of Quality Improvement

Recommends, develops, and implements department work plans, policies and procedures related to daily workflow of the Care Coordination team

Carries out key administrative responsibilities to steward care coordination program goals

Performs follow-up duties and assists members of the Care Coordination team to perform follow-up services in order to ensure that productivity, quality and customer service standards are met

  • Supports the agency's mission and all the reproductive health services offered including family planning services, health screenings, testing and treatment of STI's, contraceptive methods and abortion services.

  • Works well with volunteers and interns to achieve the agency's mission. May supervise interns and volunteers.
