    Work Shifts

    8:00 A.M - 4:00 P.M

    Job Description

    1) Complete concurrent reviews of patient records to evaluate documentation to assign the principal diagnosis, pertinent secondary diagnoses, post admission complications and procedures for accurate Diagnosis-Related Group (DRG) assignment and Case Mix Index (CMI), risk of mortality, and severity of illness.

    2) Queries physician for incomplete, inconsistent, unclear or conflicting documentation to clarify and resolve conflicting information in the medical record prior to discharge and or billing.

    a) Maintains a record of review and query activities and other appropriate records.

    b) Reconcile DRG mismatch CDIS Staff prior to submission of cases for billing.

    c) Communicate with CDIS & Validation Staff on clarification of clinical criteria needed to support documentation in the record.

    3)Attends and/or participates in staff departmental and interdisciplinary meetings, process improvement events, training and quality assurance/performance improvement activities.

    4) Performs other related work, as assigned by supervisor.

    Minimum Qualifications
    1. High School diploma or its educational equivalent; and
    2. Completion of a coding, medical record technology or health information technology
    program approved by the American Health Information Management Association
    (AHIMA); or
    3. Possession of a valid certificate as a Certified Coding Specialist (CCS) from the
    American Health Information Management Association (AHIMA); or
    4. Two (2) years of satisfactory experience in coding and abstracting medical records in a
    recognized hospital or health care organization.
