The ideal candidate must be able to demonstrate the ability to provide supportive services for youth and their families. Must demonstrate the ability to maintain contact with the youth, family and their support system. Must demonstrate the ability to produce a progress note in the CYBER and agency HER systems.
Must demonstrate the ability to generate and maintain appropriate documentation as noted by chart review with at least 98% accuracy.
Must demonstrate the ability to access local networks of services as dictated by the current ICP. Must demonstrate the ability to maintain a professional relationship with youth, family, their support system and all service providers. Must participate in and pass all mandatory trainings as scheduled. Must demonstrate basic computer knowledge.Responsibilities:
