The Substance Abuse Case Manager will play a crucial role in assessing, planning, implementing, and evaluating substance abuse treatment plans for our residents.

This individual will work closely with residents, their families, healthcare professionals, and community resources to ensure the delivery of high-quality, individualized care.

• Conduct comprehensive assessments of residents with substance abuse issues to determine their treatment needs and develop personalized care plans.
• Collaborate with interdisciplinary teams to integrate substance abuse treatment into residents' overall care plans.
• Provide counseling and support services to residents struggling with substance abuse, including individual and group therapy sessions.
• Monitor residents' progress in treatment and adjust care plans as needed to ensure effectiveness.
• Educate residents and their families about substance abuse, addiction, recovery, and relapse prevention strategies.
• Coordinate referrals to external substance abuse treatment programs, support groups, and other community resources.
• Maintain accurate and uptodate documentation of residents' treatment plans, progress notes, and related information in compliance with regulatory requirements.
• Advocate for residents' needs and rights related to substance abuse treatment within the facility and in the community.
• Stay informed about current trends, best practices, and evidencebased interventions in substance abuse treatment and incorporate them into practice.
• Participate in ongoing training and professional development activities to enhance knowledge and skills in substance abuse treatment.

• Bachelor's or Master's degree in Social Work, Counseling, Psychology, or related field.
• LSW (Licensed Social Worker) or equivalent
• Certified Alcohol and Drug Counselor (CADC) or similar certification preferred.
• Previous experience working in substance abuse treatment or related field, preferably in a long term care setting.
• Strong understanding of addiction, substance abuse disorders, and treatment modalities.
• Excellent communication, interpersonal, and counseling skills.
• Ability to work collaboratively as part of a multidisciplinary team.
• Empathy, compassion, and nonjudgmental attitude towards individuals struggling with substance abuse.
• Knowledge of relevant laws, regulations, and ethical standards governing substance abuse treatment.
• Proficiency in documenting and maintaining accurate records.
• Flexibility, adaptability, and willingness to work with diverse populations.
