Qualifications:
Bachelor's degree in social work, psychology, or related field

OR
3 years of experience within the healthcare field, case management, social work field, LTSS, Home Health or Managed Care with high school diploma.

Main Function:

Service Coordination is the process of identifying, coordinating, and facilitating all necessary supportive services and community resources for the elderly and/or disabled participants based on their individual needs.

Responsibilities:

Attend and complete forty (40) hours orientation training (1 Day in office, rest remote);
Complete annual 20-hour training;
Attend weekly meetings and trainings with supervisor (online);
Complete a person-centered assessment(HCBS* visits and remote work at home);
Develop a service plan for each participant based off of their assessed needs;
Complete monthly monitoring telephone calls and quarterly face-to-face visits;
Develop and modify the participant's service plan at least annually;
More frequent calls or home visits if necessary, to ensure the participant's health and safety is not at risk;
Responsible to maintain ongoing communication with Supervisors in regards to a participant's case;
Maintain outgoing communication with participant and providers;
Notify the participant of the participant's right to choose any willing and qualified provider to provide a service on the participant's service plan;
Participate in remediation activities;
Ensure a participant exercising participant-directed budget authority does not exceed the number of service hours approved in the service plan;
Report incidents and allegations of neglect/abuse to Supervisor and Administrator;

**Must have a valid and current drivers license.

**Needs reliable transportation and flexibility in traveling: We Serve The Following Counties:

Southeast Zone - Bucks, Chester, Delaware, Montgomery And Philadelphia Counties. Sometimes other surrounding counties.

Bilingual individuals always a plus.

*HCBS = Home and Community Based Services

Pay:
From $20.00 per hour
