    Patient Navigator Coordinator - New York, United States - NueHealth

    Description

    The Patient Navigator will be responsible for building a working relationship with patients by guiding them through the healthcare system and safeguarding their healthcare needs. This position will also oversee the pre-admissions testing procedure while ensuring medical clearance measures have been met.

    Minimum Qualifications

    • High School Diploma or GED degree required
    • Minimum of 1 year experience in clinical health setting required
    • Excellent customer service skills
    • Working knowledge of pre-admissions procedures
    • Working knowledge of medical clearance procedures
    • Working knowledge of medical terminology
    • Ability to multi-task with time management skills
    Essential Functions
    • Schedules anesthesia medical clearance
    • Updates pre-op call list
    • Obtains pre-op testing from surgeon or primary care physician office
    • Manages infection control reports sent bi-weekly post-operative to surgeons office for completion
    • Manges implant tracking report sent every 6 months and one-year post-operative for all patients who received and implant
    • Serves as a liaison between patient and staff in a courteous and professional manner
    • Identity and problem solve any service gaps identified
    • Creates a seamless flow through the health system to facilitate the patients care
    • Accommodates interpretive language services if needed
    • Works collaboratively with all members of the healthcare team to meet the patient needs

    While performing the duties of this job the employee is frequently required to sit, converse, and listen; use hands to touch, handle, or feel objects, tools, or controls; and to reach with hands and arms. Specific vision abilities required by this job include close vision and the ability to adjust focus.

    The employee must be able to lift and/or carry over 20 pounds on a regular basis and be able to push/pull over 25 pounds on a regular basis.

    The employee must be able to stand and/or walk at least five hours per day.
