Job Summary:

The Patient Navigator provides case management services to the patients in the nephrology clinic. The incumbent will be the main point of contact for new and established patients with Polycystic kidney disease.

The Patient Navigator builds strong relationships with Polycystic kidney disease patients to help them stay engaged in medical care and adhere to their medications.

Patient Navigator is committed to removing the patient's barriers to care by identifying critical resources, helping them navigate through health care services and systems, and promoting health.

They work closely with the Care Team including doctors, nurses, and other clinical staff to support positive health outcomes


Responsibilities:


  • Providing support to patients, including scheduling patient appointments with providers
  • Helping with gap closures: monitor care management programs' phone line, triaging patients to appropriate care managers and providers to fill gaps in care.
  • Managing relationships with referring provider, transitions care from diagnostic services to procedural services as required.
  • Obtain clinical information from other integrated medical systems as necessary.
  • Inquire about medication usage, refills, and REMS enrollment.
  • Respond to routine inquiries, record, and escalate issues as appropriate.
  • Identifies resources for patients to overcome barriers to care, such as transportation, housing, and insurance
  • Obtain all required registration and intake information from patients necessary for an efficient checkin process; verify and/or update any new insurance eligibility, benefits, or other information prior to the start of the patient appointment in the electronic health record (EHR); accurately indicate arrivals, cancellations, and noshows in the EHR.
  • Collect all timeofservice and past due payments prior to the start of the appointment. Settle cash drawer in the EHR daily
  • Track patients' information, schedules, files, and forms in a confidential manner
  • Assist with patient's paperwork for transition to dialysis.
  • Attends and represents the organization at professional conferences, inservice training, and meetings at the request of or with the approval of supervisor
  • Identify potential patients for clinical trials
  • Perform other duties as assigned

Job Qualifications:


  • High school diploma or GED equivalent required.
  • Commitment to the mission of care coordination
  • Strong understanding of cultural competency
  • Passionate, trustworthy and empathetic when working with patients
  • Ability to build relationships with different types of people including patients, organization members and health care providers
  • Excellent communication and interpersonal skills and ability to speak concisely to patients and Care Team members
  • Organized with confidential patient material and appointment tracking
  • Flexible and adaptable in response to changing needs of patient and health care providers
  • Interest in working with kidney patients
  • Bilingual (English/Spanish) preferred
**Hourly Rate Ranges: $ $33.17
