    Description:

    Position Summary
    This is a Tier IV Extra Help Discharge Planning HURC II Position. This position requires 32 hours of on call availability per 4 week schedule and must arrive within 1 hour of call. This position coordinates and provides discharge planning. There are a total of 2 vacancies. Working hours are 8am to 430pm. The department typically needs coverage on Mondays, Fridays, weekends, and other occasional weekdays.

    Duties & Responsibilities
  • Coordinates and provides discharge planning. Screens patients for high risk discharge planning needsand assures collaboration with interdisciplinary team members.
  • Documents risk assessment and discharge planning information in the medical record according to department standards.
  • Collaborates with the patient, family, physicians, RNs, and multidisciplinary team to assure appropriate safe, cost effective, timely and efficient services. Utilizes clinical and social work experts as needed to ensure delivery of comprehensive patient services.
  • Facilitates daily communication (e.g. patient rounds, multidisciplinary team meetings and conferences) with the interdisciplinary team to discuss plan of care and assist with expediting care across the continuum.
  • Facilitates coordination of patient services in assigned patient care unit, or department.
  • Qualifications:

    Minimum Qualifications
    1. Graduation from an accredited professional nursing program
    2. Current licensure as a Registered Professional Nurse with the Illinois Department of Financial and Professional Regulation, Division of Professional Regulation.
    3. Five years (60 months) of RN experience.
    4. Current AHA BLS/CPR and other required unit specific certifications
    5. Effective communication skills (oral and written) that facilitate therapeutic relationships between patients, family, and health care providers
    6. Intermediate computer skills with the ability to become proficient in the Electronic Health Record (EHR) with on-the-job training
    7. Excellent attendance, interpersonal skills and work history required.

    Hospital Case Management/Discharge Planning Experience Strongly Preffered
