- Responsible for the primary care of assigned patient panel across the continuum of care.
- Coordinates patient care through patient management techniques within the assigned Patient Aligned Care Team (PACT).
- Maintains effectiveness in recognizing emergency/acute situations and intervening in an appropriate and effective manner.
- Documents patients health care clearly, adequately and accurately as defined by medical records documentation, chronic disease and prevention indicators.
- Signs notes in a timely manner as dictated by Medical Center by-laws or the Primary Care Service Chief.
- Ensures that levels of supervision are documented in the electronic medical record according to Medical Center policy.
- Identifies opportunities in clinical practice that could be improved through performance improvement activities.
- Manages patient workload and wait times to assure the most effective, efficient service to the Veterans.

Summary
This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
