    Planned Parenthood Los Angeles is seeking an experienced Case Management Specialist with excellent communication and customer service skills to work in our Downtown Los Angeles Headquarters.

    Provide high quality Patient Case Management services targeting PPLA patients. Review each follow-up plan assigned and work the follow-up template.

    Our Ideal Candidate will have the following qualifications:

    High School Diploma or equivalent requiredOne (1) to two (2) years health care experience, preferably in a medical or public health setting requiredDemonstrated proficiency in EPM (enterprise practice management) and EHR (electronic health records) requiredReliable and flexible means of transportation is requiredBilingual English/Spanish preferredSupportive of the mission and goals of Planned ParenthoodAbout the position:Abortion patients are cared for at each of our health centers, and in part through the administrative, support, and other non-clinical services provided at all PPLA locations, and by all PPLA employees.

    Supporting these critical services is an essential job duty, and a fundamental responsibility of all employees and contractors.
    Maintain ongoing and appropriate communication with Case Management Manager and/or Case Management SupervisorReview each follow-up plan assigned and adhere to the follow-up templateReview the Visit Summary, lab result, Med Master, Communication Template, and additional notes as applicable for each follow-up planSend Confidential Morbidity Report (CMR), if applicableMake 2nd, 3rd and reminder contact attempts within specified time frame requirementsMake appointments in EPM for patients as neededProvide medically accurate information and comply with patient notification requirements per PPFA standards, guidelines, protocols, and policiesDocument during, or immediately after, each call with accurate and clear informationMaintain accurate and current documentation in EHRPrepare for daily mail pick-up by packing letters and inserts into envelopes, as neededDocument and scan receipt of all "Returned mail"Scan patient documents into charts as neededEnsure the accuracy of patient information; use appropriate identifiersUpdate patient address and phone numbers in NextGen after identification of patient confirmedNotify Case Management Supervisor of any patient issues or complaintsSubmit a Case Management Productivity report for each day workedKeep up-to-date on any treatment, procedure, or policy changes as communicated during huddles, newsletters, or emails from Case Management and/or Patient Services leadershipComply with PPLA's Standards of Conduct and maintain the highest level of professional and ethical standards, acting in compliance with both the letter and spirit of all applicable laws and regulationsEstablish an effective working relationship with internal and external customersNotify the appropriate contact person/people when issues or problems ariseMake sound decisions and demonstrate the ability to handle stressful, sensitive, and/or challenging situations appropriatelyGenerous salary and benefits package includes:Medical, dental, and vision coverage options for you and eligible dependentsFree basic life/AD&D policy with additional voluntary coverage optionsShort Term Disability, Critical Illness and Accident policies403(b)
