- This role assists individuals in Shared Living/Adult Foster Care setups in Massachusetts.
- You will deliver case management services to the individuals in our Shared Living/Adult Foster Care Program under the supervision of the Program Director.
- Oversight and support to the Shared Living/Adult Foster Care provider will be provided.
- Bring forth your innovative ideas and enthusiasm for success; we will provide training on the remaining aspects.
- $42-$44k.

- Coordinate the planning, review, and execution of services, case management, behavior intervention plans, and treatments for the individuals served.
- Supervise all services offered to individuals and their families, ensure the quality of care provided, guarantee compliance with contract terms, and oversee regulatory program adherence.
- Contribute to the delivery of exceptional services and exciting activities tailored to meet the needs of the individuals served.
- Establish and nurture relationships with families and external case managers.
- Completion of a Bachelor's degree.
- Possession of a valid driver's license, car registration, and auto insurance.
