    We are looking for a dedicated Home Care Nurse (LPN/RN) to join our nursing team and provide top-notch care to our clients in the comfort of their homes. If you are passionate about making a difference in people's lives, this opportunity is for you.

    Details about this client's case:

    • Location: Rosemount, MN
    • Patient: 16-year-old Male
    • Diagnoses: CP, Von Willebrand's Disease
    • Conditions: Developmentally Delayed, Trach, Vent/BiPap at Night
    • Shifts: NOC shifts (9pm - 7am), E/O Weekend Shifts (Fri, Sat, Sun) 11am - 9pm

    Responsibilities:

    • Administer nursing care tailored to the client's needs
    • Assist in adhering to the patient's treatment plan
    • Observe and communicate any changes in patient symptoms or behaviors
    • Keep detailed and precise medical records
    • Offer guidance and assistance to patients and their loved ones

    Qualifications:

    • Demonstrate empathy and maintain the respect and dignity of each client
    • Hold a valid RN or LPN license in Minnesota
    • Establish and nurture relationships with patients
    • Possess strong problem-solving and critical thinking capabilities

    Salary:
    $36.35 per hour

    Position:
    Full-Time and/or Part-Time

    No prior experience required - We will provide comprehensive training to ensure both you and the client feel confident and at ease.
