- Home visitation with infants/toddlers and their families
- Conduct developmental screenings and/or eligibility evaluations
- Collaborate with clinical team, families, and other professionals
- Participate in development of the Individualized Family Service Plan (IFSP)
- Assist with the transition to preschool programming and other community supports
- Working knowledge of Microsoft Office including Excel
- Ability to develop effective relationships
- Excellent interpersonal skills
- CT Licensed/Certified Speech Therapist - MA/MS - CCC, SLP (CFY candidates considered)
- CT Certification 06- CT TRB Eligible
- Experience with Infants, Toddlers and Families
- Service Coordinator Certificate (or willingness to obtain)
- Frequent standing, bending, kneeling and therapeutic holding as required
- Must be able to perform essential functions with or without accommodations
