    Summary

    Under the direction of the Division Director, Clinical supervisor, and Program Manager, the Care Manager I / II will work with a multidisciplinary team in providing a comprehensive service to clients.

    Essential Functions

    § Conduct regular community education and promotional outreach.

    § Work collaboratively with Adult Protective Services to address immediate client risk factors

    § Evaluate clients' eligibility for service, conducting in-home assessments and coordinating service arrangements/authorizations.

    § Develop individualized care plans that will ensure a seamless system of services that is culturally sensitive and language appropriate.

    § Provide thorough and comprehensive documentation of client services and notes.

    § Explore appropriate options for short-/long-term care and supportive resources with client and family geared at maintaining client independence, autonomy and self-determination.

    § Develop and maintain a network of support for clients (e.g. through family, community groups) through case monitoring.

    § Develop and maintain community relationships with public and private service organizations, both ethnic and mainstream.

    § Maintain a client case load ratio of up to a maximum 40:1.

    § Work with multidisciplinary mental health team in providing 24/7 crisis intervention.

    § Availability to work some evening and weekend hours as necessary.

    § Participate in monthly Care Plan Team Conference and other group supervision.

    § Responsible for all monthly paperwork, data gathering and project billing specific to client caseload

    § Advocate for issues relating to and affecting the population served.

    § Driving to and from meetings, client visits, and other events requiring travel throughout LA County.

    § Other responsibilities as assigned by the Division Manager and Program Coordinator.
