Summary:


  • Under the direction of the Lead Case Manager, the Home Safe Case Manager is responsible for completing enrollment, intake, service coordination, and triage for older adults seeking services through the Home Safe Program._

Housing Navigation Duties:


  • Establish a housing network by developing links with landlord and property management companies
  • Unit viewing and inspection
  • Create and manage data of available rental units in the Housing Stock Portfolio
  • Develop partnership with landlords and property management companies to give priority to HOPICS Rapid Re
  • Housing tenant referrals
  • Requesting the security deposit for the participant's identified unit
  • Requesting TLS for participants housing subsidy support months)

Essential Navigator Functions:


The Case Manager will have a case load up to 20 clients for up to 18 months maximum

  • The Home Safe Case Manager will be required to meet with the participant and execute the following functions: intake and enrollment, housing identification, case management, and assist with financial assistance requirements from enrollment to exit into permanent housing or housing stabilization.
  • Staff must create the participant's record in HMIS (If not currently enrolled) at the same time as the participant is determined to meet program eligibility requirements within 24 hours.
  • Verify participant meets Eligibility for service requirements.
  • Provide a housing-first model to assist participants in becoming self-sufficient and obtaining permanent housing through referrals.
  • Will collaborate with participant to assure client is document ready for housing
  • The Case Manager will screen participants in Problem-Solving to determine if there is access to other housing options and support.

Essential Case Management Functions:


  • Case Manager will collaborate with the client to complete an individualized housing plan and budget to ensure the household can sustain housing after the subsidy.
  • Case Manager may be required to meet with the participant at their home, at an encampment, or at another program.
  • Conduct case management meetings with participants at least twice a month.
  • Complete Exit Plans for all participants assigned to the case manager.
  • All case notes for services rendered must be entered into HMIS within 24-48 hours of service, in GIRP format.

Case Manager will meet with the Lead Case Manager for weekly supervision and monthly group supervision sessions

Must attend weekly case conference meetings with community partners.

  • Maintain up-to-date and accurate documentation in client files (Dropbox) for service coordination/case management, housing placement, rental assistance, and move-in assistance.
  • Provide the Lead Case Manager with all required client information and assessment outcomes.

Ensure compliance with HIPAA, 42 CFR Part 2, and other funding requirements.

  • Maintain appropriate boundaries; and adhere to AB's Code of Ethics and AB's Core Values.
  • Represent the Agency in a professional manner at meetings, training, and community events.

Maintain a high level of ethical conduct regarding confidentiality of employees, participants, company, vendor's data, and manage sensitive and critical information with professionalism and discretion.

  • Always maintain a safe environment, complying with Agency and industry standards and safety regulations.
  • Perform other duties as assigned or needed.

Requirements and Qualifications:


  • A minimum of one (1) years' experience working in HMIS (preferred).

If in recovery, a minimum of three (3) years of being drug and alcohol-free is required

  • Must have a working knowledge of the intake process for homeless program services.
  • Working knowledge of Microsoft Word, Excel, Google, and other database programs.
  • Knowledge of resource development, and navigating CPS, DMH, DPSS, LAUSD, Work Source, SUD providers, and
