Position Overview:
As a Case Manager at WLCAC Senior Services your primary responsibility will be to conduct in-home and telephone assessments for senior citizen clients.
You will also perform initial and quarterly assessments of Home Delivered Meals Program participants and provide essential administrative support to our Case Manager.
This role involves conducting client intakes, follow-ups, data entry, filing paperwork, outreach, and more. Supervise the case management department.Key Responsibilities:
- Conduct inhome and telephone assessments for senior citizen clients.
- Perform initial and quarterly assessments of Home Delivered Meals Program participants.
- Conduct client intakes and followups.
- Interact with clients over the phone.
- Data entry and recordkeeping.
- Filing paperwork and maintaining organized records.
- Outreach to clients and community resources.
Qualifications:
- Bachelor's degree in social work or a related field preferred.
- Valid driver's license and reliable transportation.
