Job Summary
The Intensive Case Manager position is part of the Behavioral Health Services team at the 1930 Market Street Service Center and is responsible for providing case management and substance use services to individuals and groups who have severe mental illness and substance use issues and/or are living with HIV/AIDS.
This position is part of a larger multidisciplinary behavioral health services team that includes psychiatrists, nurses, social workers, therapists, substance use counselors, case managers and psychologists.
The person in this role works on a program of Intensive Case Management services under the "Getting to Zero" Initiative, working to help HIV-positive clients navigate the multiple psychosocial stressors which have kept them non-engaged and non-compliant with medical care and assuring their ongoing health and well-being.
This position provides case management services to individuals living with co-occurring diagnoses of substance use issues and severe mental illness, within a Harm Reduction philosophy, and utilizes Motivational Interviewing, among other interventions.
This position will involve providing intensive clinical case management services to clients who have been non-engaged in their medical care (particularly HIV medical care) and addressing the psychosocial barriers to clients engaging in, and maintaining, their treatment adherence.
This position will involve field work and engaging clients in their home as well as community settings, and working collaboratively with collateral service providers to ensure continuity of care.

The Alliance Health Project (AHP) is a community-based LGBTQ+ and HIV-focused clinic located at 1930 Market Street in San Francisco.
We provide psychotherapy, substance use counseling, case management, support groups, psychiatry, sexual health services, clinical traineeship, and provider education and training.
Our multi-disciplinary staff are a client-centered, collaborative, Harm Reduction-oriented team.We have continued to grow as a community-based clinic by anticipating and addressing the changing needs of our LGBTQ+ and HIV community members for more than 35 years.
Our process of continuous improvement of service to our community includes a strong focus on cultural humility and anti-racist service provision.
Required Qualifications
Possession of a bachelor's degree in social work from a program approved by the Council on Social Work Education, or related field of Social Service, Counseling, Psychology and one year of experience; or an equivalent combination of education and experience; and knowledges and abilities essential to the successful performance of the duties assigned to the position.
One year of experience in assessing and managing the psychosocial needs of persons with HIV/AIDS and with persons identifying as LGBTQ, including case management and substance use counseling.
One year of direct clinical experience in the treatment of multiply diagnosed clients (mental health conditions, substance use, medical issues, etc.).
Preferred Qualifications
Fluent in Spanish, with written and verbal proficiency and past professional experience utilizing Spanish in clinical work.
Understanding of psychosocial issues facing the LGBTQ and HIV-affected communities.
Experience working within Harm Reduction treatment models, utilizing Motivational Interviewing interventions
Familiar with hospital-based case management services, linkage and/or intensive case management services
One year of experience working within a multidisciplinary team.
One year of experience coordinating care with service providers in a collaborative, care-coordination capacity
Sensitivity to working with cultural, ethnic, and racial issues.
Understanding and experience working with severe mental illness.
Ability to work independently in non-office based and community settings in intensive case management work
Excellent communication and interpersonal skills.
Demonstrated excellence in reliability and attendance.
License/Certification
Within 1 year of employment, successful completion of San Francisco Department of Public Health Involuntary Hold 5150 certification training, and maintaining continuous certification.
