    Program Description

    The Felton's Harbor Haven Supportive Housing Program Case Managers will be responsible for a caseload of 20-25 program participants that meet the HSH/HUD qualifications for the program.

    CM's will be required to check-In or meet with participants assigned to their caseload once a week or as needed to make sure case management needs are being meet, and linked to outside resources and services, CM's will be required to conduct Initial and Annual Assessments and Treatment Plans of Care and writing daily standard Progress notes within Felton EHR Chart Health System Circe and HSH One System.

    The supportive Housing Program helps to increase coordination among its service delivery while supporting clients in navigating transition of care, supervision, and services for individuals; and improve client outcomes to reduce recidivism and promote self-efficacy.

    CM's will be committed to providing community-based transitional case management services for up to 12 months for program participants while linking them to other support services.

    Program Specific Responsibilities
    Works closely with an interdisciplinary team of case managers and community providers to support clients.
    Provides Intervention and case management to reduce decompensation and/or prevent homelessness.
    Completes intakes and Initial/Annual Assessments and Treatment Plans of Care.

    Provides case management services, including evaluations, assisting clients in accessing permanent housing, financial, medical and other needed services while Coordinating with other service providers.

    Maintains records (Progress Note) after each client contact, in accordance with HSH/HUD requirements.
    Participates in team staff meetings, and any other meetings required by HSH/HUD.
    Commits to ongoing training by participating in Felton's required trainings.
    Provides information regarding community resources to program participants as needed.
    Provide transportation as necessary to identified participants who require assistance in navigating in the community.
    Other duties and responsibilities that may be assigned from time to time.

    Minimum Qualifications:
    A minimum of 3 years of case management experience
    Demonstrated ability to utilize computers and proficiency in Microsoft Office software.
    Ability to work with clients with various ethnic backgrounds
    Ability to manage and maintain strong structure and boundaries when transporting clients in a motor vehicle.
    Valid CA Driver's License
    Valid Motor Vehicle Insurance
    Strong organizational skills
    Strong communication skills both verbal and written
    Strong interpersonal, organizational, and time management skills
    Ability to be a team player with a welcoming attitude
    Must be able to lift-up to pounds.
    Ability to follow instructions from management
