RN Case Manager

Responsibilities:


  • Provide case management services
  • Develop and implement individualized care plans for clients
  • Monitor and evaluate client's progress
  • Provide crisis intervention and support
  • Collaborate with other healthcare professionals to ensure quality care
  • Maintain accurate and uptodate client records
  • Home visitation as needed
  • Other duties as assigned.

Qualifications:


  • Registered Nurse (RN) license in the state of California
  • Minimum of 3 years of experience in case management
  • 1 year of working with the geriatric population
  • Baccalaureate degree in nursing.
  • Bilingual preferred
