Participants and their counselors work collaboratively to design an integrated, individualized, strengths-based treatment experience geared towards addressing each client's specific needs and goals.

Outpatient programs provide Individual and group therapy, individualized case management, treatment planning, drug and alcohol counseling, crisis intervention, and connection to social support services.

Job Overview

HealthRIGHT 360 has provided outpatient services for behavioral health disorders since 1985.

ADAPT is specifically designed for people who struggle with substance use disorders, mental health issue/co-occurring disorders, and criminal justice involvement.

ADAPT works with high-risk populations from a variety of backgrounds providing trauma-informed care aimed at reducing the harmful impacts of substance use.

Our programs are open and welcoming to adult participants of any gender identity.

We accept the majority of participants who seek Outpatient treatment, including walk-in's, referrals from other agencies, and step-down transitional clients from residential treatment programs.

Participants and their counselors work collaboratively to design an integrated, individualized, strengths-based treatment experience geared towards addressing each client's specific needs and goals.

The Case Manager is responsible for assessing treatment needs; monitoring and evaluating problem lists and progress and facilitating interdisciplinary approaches.

The Case Manager coordinates an approach to the delivery of health, substance use, mental health, and social services, linking clients with appropriate services to address specific needs and achieve stated goals.

The Case Manager's duties include care coordination, maintaining positive working relationships with outside community partners, scheduling appointments, arranging transportation, assisting with acquiring medications, ongoing care and discharge planning, and crisis intervention.

Key Responsibilities

  • Help clients get and stay engaged in SUD treatment.
  • Help clients transition between levels of care.
  • Create a safe space for clients and maintain appropriate boundaries.
  • Interface with a multidisciplinary team in treatment planning and service delivery.
  • Develop and assess the effectiveness of individualized treatment and consumer progress.
  • Assist in the ongoing maintenance of consumers' charts and other related documentation in compliance with agency, HIPAA, and DPH standards.
  • Provide case management services to clients as assigned.
  • Assist client in developing independent living skills to promote independence and selfsufficiency.
  • Provide "whatever it takes" services to consumers including providing services where the consumer lives, assisting with housing, benefits, employment, education, transportation, childcare, medical and other needed services.
  • Serve as a care coordinator for clients as assigned.
  • Responsibilities include coordination of all services with other providers and completion of all coordinated care documentation.
  • Assist in crisis and symptom management.
  • Assess for opioid overdose and ensure beneficiary has Naloxone if appropriate.
Education and Knowledge, Skills and Abilities


Education:


  • Must be SUD registered or certified with a credentialing agency
  • High school diploma or GED required.
  • Valid CA Driver's License.
  • First Aid Certified within 30 days of employment.
  • CPR Certified within 30 days of employment.

Background Clearance Required:


  • Must not be on active parole or probation.

Knowledge Required:


  • Some experience providing case management services is required.
  • Culturally competent and able to work with a diverse population.
  • Knowledge of housing, employment, SSI resources, and success with linking clients.
  • Experience working successfully with issues of substance abuse, mental health, criminal background, and other potential barriers to economic selfsufficiency.
