June 11, 2024JOB SUMMARYThis position is responsible for initiating and leading the development of supports and training in collaboration with local agencies and community organizations to increase community access for board eligible people.
This position will be part of the Early Childhood area.
ESSENTIAL DUTIES AND RESPONSIBILITIESEvaluation of this position is based primarily on performance of the following essential functions, which include, but are not limited to: 1.Evaluate and identify barriers/limitations to community access.
Design and/or provide the supports necessary to increase access for board eligible people to achieve their desired lifestyles and increase their capacity of independence, socialization and productivity in their communities.2.Initiate and maintain proactive relationships with community partners and stakeholders to actively support, practice and create opportunities for board eligible people.3.Serve as projects leader as determined by supervisor.
Role includes, but is not limited to, gather information, determine outcomes/goals, formulate strategies to support successful access/opportunities for board eligible people, coordinate team, research, design and facilitate trainings/presentations internally and externally.4.Assist internal staff and external organizations through knowledge-sharing, co-creating strategies and modeling best-practices.
5.Professionally collaborate and represent Summit DD in all interactions with internal and external staff/customers, community partners, board eligible people, families and community members.6.Represent Summit DD on community committees to ensure events and activities are available and accessible to all board eligible people.7.Follow safety and health rules and regulations, including, but not limited to OSHA standards.8.Fulfill additional duties/responsibilities appropriate for the position and required for the safe operation of the program.
Education:
Bachelor's degree in Early Childhood, Special Education or related field from an accredited university or college
Experience:
Three years of verifiable paid work experience working with children or adults with developmental disabilities Developing and/or implementing community based services focused on supporting greater community access and the building of natural supports
Skills:
Exceptional customer service, provider relations, communication, negotiation and interpersonal skills required for interactions with internal and external agency staff.
Strong leadership and decision making skills required. High level of self-direction and accountability for actions required.KNOWLEDGE, ABILITIES AND OTHER EXPECTATIONSKnowledge:
Programming techniques and therapeutic interventions for children and adults with developmental disabilities, with a focus on techniques that support community access.
Ability to work with individuals who may present a variety of services and support needs. General principles and objectives of information technology systems and programs.Proper spelling, grammar, punctuation, and sentence structure to ensure that written materials prepared and reviewed are complete, succinct, and free of errors.
Procedures and protocols to prepare meeting minutes.Abilities:
Complete data entry, generate reports, copy, manage and track documents and information.
Effectively manage time to meet operational needs and desired outcomes, and plan and organize the steps to implement new projects.
Listen to, understand and communicate information and ideas.Work independently with minimal supervision.
Maintain strict confidentiality.
Work as part of a team and work effectively with persons and communities from diverse cultural, social, and ethnic backgrounds.
Work effectively under pressure.Other Expectations:
Report unusual incidents and major unusual incidents. Completes all required training and maintains strict confidentiality. Regular attendance is mandatory.
Maintain related records in accordance with appropriate records retention guidelines and policies.
Adhere to health and safety policies and procedures.
Must have reliable transportation to fulfill responsibilities requiring travel in the community. Must be available by personal cell phone during working hours.
