- Leads the Logistics teams in the coordination of patient movement and bed assignments, based on appropriate level of care and patient aggregation from health-system Emergency Departments, Perioperative Services, Direct Admissions, patient transfers, within the hospital and transfers from other hospitals identified by the Transfer Center, for health-system admission.
- Has authority to lead teams in lieu of the Chief Operating Officer of The Clinical Command Center of the MSHS to insure projects requiring coordination of multiple services and departments result in successful completion. Acts as designee for the COO of the CCC to approve diversion decisions.
- Devises strategy and interacts with CMOS, CNOs and COOs of system hospitals on the placement of patient populations in 8 hospitals and over 95 inpatient units. Promotes optimum utilization of system hospital beds by conducting rounds and communicating with admission planners, admitting staff, Emergency Department, Perioperative Services, Transfer Center and clinical and administrative staff on the patient care units regarding bed availability, current census, admission and throughput needs.
- Facilitates MSHS multidisciplinary huddles, to review and expedite patient movement across all hospital campuses. Clinically reviews patient charts and works with MSHS Medical, Nursing, Infection Prevention, Transport, and Environmental Services departments to validate infection/isolation protocols for safe patient placement.
- Leads and interacts with senior leadership within the health system to level load patient populations, with a working knowledge of every service line, for every level of care, from ICU to med surg, across the health system. Intervenes in conflicts as needed. Liaises with Nursing and Providers to facilitate patient discharges, inter-facility and intra-hospital transfer processes.
- Executes the goals and objectives of the MSHS Clinical Command Center's Throughput, Transfer Center and Telemedicine divisions that are consistent with Mount Sinai Health System or School of Medicine's policies and federal, state and local regulations. Advises senior leadership of operational issues related to department activities within all divisions.
- In conjunction with health-system Nursing, Medical, Infection Prevention and Quality Leadership, coordinates performance improvement activities within designated clinical specialties, that include but are not limited to: the verification of isolation and infectious precautions patient populations, constant observation and patient restraints review, risk management activities identifying and communicating issues of risk, discharge planning and follow-up, patient and family education and quality improvement programs.
- Partners with all clinical institutes i.e., Critical Care Institute, in the management of critical care patient throughput throughout the Mount Sinai Health System.
- Develops and designs action plans to support performance and provides support and educational resources to departments and services for optimal care delivery, across MSHS. Demonstrates the ability to work with all clinical and non-clinical teams and displays critical thinking for comprehensive health-system bed management, transfer services and global patient throughput.
- Oversees the collection, compilation, and analysis of all health-system throughput activity data to evaluate operational performance and competence. Coordinates 6AM Census, Throughput, Transfer Center and Telemedicine updates. Addresses and resolves daily issues arising from increased census and decreased bed availability.
- Oversees and collaborates with Nursing, Support Services, Ancillary Services, Clinical IT, Environmental Services and more to ensure appropriate discharge timing and throughput. Leads patient inter and intra hospital flow decisions and surge planning. Coordinates active engineering projects across the health-system.
- Liaises with Nursing, Providers and interdisciplinary teams to facilitate patient discharges, inter-facility and all transfer processes throughout the health-system. Follows up on patient concerns in a timely manner and assists with service recovery as appropriate. Considers the patient's preferences, cultural diversity and expressed needs in all aspects of the patient movement process. Conducts quality and safety investigations, prepares and participates in the RCA process and establishes and implements process improvement plans.
- Coordinates accurate, current/up-to-date census and patient bed availability information for all system hospitals. Acts as designee for the CMO of MSH to report situational statistics.
- Oversees 24 hour patient care delivery, throughput, transfer center and Clinical Command Center staffing and processes to ensure efficient and effective operations. Serves as a clinical role model and professional resource to staff and other members of the health care team. Collaborates with care teams, across the health system, in the development, implementation, and ongoing review of policies, procedures, and standards of care.
- Aware of and responsive to budgetary constraints and incorporates financial principles into program development and allocation of resources. Acts as designated authority for financial authorizations in lieu of insurance approval on off hours.
- Recruits, orients, trains, coaches, counsels, mentors, disciplines, and evaluates staff in accordance with all internal policies and procedures. Manages affected team members in compliance with collective bargaining agreement.
- Conducts regular staff meetings to ensure communication with staff about progress, changes and compliance with health-system policies and procedures. Daily reviews and assigns tasks and responsibilities, plans, monitors and appraises job performance. Huddles with staff daily, at the start of every shift, to identify high-risk patients and situations that require special attention. Attends and facilitates health system huddles to receive and report situational awareness across the health-system.
- Assures readiness for Joint Commission and other agency surveys. Identifies and investigates issues or variances in practice/operations by participating in the RCA process and corrective action plan implementation across the health-system.
- Serves as preceptor for new leaders and other staff. May work variable shifts/hours and weekends, within the Clinical Command Center, to meet the changing needs within any/all MSHS hospitals.
- Other duties assigned as required.
- Must be a Licensed Registered Nurse, Physician Assistant or Nurse Practitioner
- Five (5) years of clinical experience in a hospital setting
- Two (2) years of critical care and/or emergency department experience preferred
- Two (2) years of management or supervisory experience preferred
- One (1) year of Patient Flow experience preferred
- EPIC experience preferred.

The Clinical Program Manager, Clinical Command Center participates in the coordination of global clinical programs for designated patient populations, collaborates with health system clinical teams to facilitate patient admissions, discharges, intra-hospital and inter-facility transfer processes.
Responsible for assuring compliance with mandated clinical interventions inclusive of core measures within all system hospitals and entities.Oversees daily Mount Sinai Health System (MSHS) activities, which includes but is not limited to, policy and program development, staff training, patient assessments, patient throughput, managing patient emergencies and supporting patient-centered care.
