    Description
    The UVA Department of Psychiatry and Neurobehavioral Sciences seeks candidates for a Case Manager/Helpline Coordinator position.

    This individual will be responsible for assisting staff and working with patients and families to provide support and promote care coordination and system navigation between health care providers and community agencies and resources based on staff recommendations.

    Also, will be responsible for coordination of the statewide substance use helpline.

    The incumbent must have a positive customer-focused attitude and enjoy contributing significantly to team and project goals.

    Candidate assists with problem solving of complex patient care issues which includes:


    • Provide support and assistance to patients and families during the intake process. Assists patients and families in being proactive regarding their client service plans.
    • Communicate with the clinical team regarding status of referrals, outreach, and care coordination efforts.
    • Provide care coordination and resource navigation services using a patient- and family-centered approach.
    • Assist patients and families in learning to communicate effectively with health care educators and other service providers.
    • Assist patients and families in identifying and prioritizing their needs.
    • Identify and address service gaps and barriers that make it difficult for patients and families to meet their health care needs.
    • Compile and maintain up-to-date lists of local and statewide resources as appropriate.
    • Collect, update, and/or create resource materials to foster understanding of addiction patient care needs.
    • Compensate patients according to SOP through our social grant program.
    • Develop and maintain ongoing connections with community resources, support groups and other service providers.
    • Collaborate with community partners to share current resource information and advocacy approaches.
    • Facilitate patient enrollment and participation in any research or other relevant health care initiatives, as appropriate.
    • Investigates, documents and resolves or refers as appropriate problems and conflicts.
    • Constantly and consistently evaluates processes, identifies opportunities for improvement and offers constructive ideas and solutions; actively participates in solutions and implementation.
    • In addition to the above job responsibilities, other duties may be assigned.

    MINIMUM REQUIREMENTS

    Education:
    Bachelor's Degree


    Experience:
    Two years progressive work experience, including experience working with complex patient populations, medical terminology, and customer service.


    PHYSICAL DEMANDS
    This is primarily a sedentary job involving extensive use of desktop computers. The job does occasionally require traveling some distance to attend meetings, and programs.
