    JOB DESCRIPTION

    SUMMARY

    Under the direct supervision and guidance of the MHRS Clinical Manager, the Clinical Care Coordinator (CCC) will serve as the liaison between the MHRS team members, to promote efficacy of services and quality of care. The CCC will work closely with the Clinical Manager, Community Support Workers, and Team Leads within MHRS, and models a teaming approach to communication across departments and with stakeholders.

    The CCC responsibilities will include managing the clinical consultation of the community support team that they are assigned to. The CCC with support of the Clinical Manager will ensure referrals and wrap around care are provided from initiation to discharge. The CCC assists with completing intake assessments, managing admission of program consumers and maintaining the program's census and consumer discharges. The CCC adopts a "whole-person" approach to address the client's needs related to physical health, behavioral health, and social determinants of health.

    CCC involves coordination of care between the behavioral health clinician and the clinical personnel of an external provider (e.g., primary care, another behavioral health provider, hospital). He/she will also ensure the completion of all clinical documentation and work cooperatively with other team members to ameliorate the symptoms of those assigned to his or her caseload, with treatment driven by the person centered treatment plan. Independent judgment is required to plan, prioritize and organize diversified workload.

    ESSENTIAL FUNCTIONS

    1. Prestige Clinical Care Coordinator will work under the direction of the team Clinical Manager, and by extension, the Clinical Director of MHRS, to oversee the operations and execution of person-centered treatment on the community support team.
    2. The CCC shall ensure appropriate consumer referrals, authorizations, and transitions to new levels of care (LOCs). Additionally, the CCC will ensure follow-up and coordination of services and actual connection to other services for consumer. If a consumer fails to make progress or has met all of their treatment goals, the CCC shall ensure timely assessment and transfer to a more appropriate LOC. This is done in support of the primary community support worker assigned.
    3. Will facilitate managing day to day referrals to and from the program, scheduling court hearings and follow up with CSOSA/Probation or Parole officer if consumer is mandated to receive outpatient treatment, community resource management and data tracking. The CCC is also an integral member of the Treatment Team(s) when the Community Support Worker is unavailable, and to provide team support.
    4. Identifies community resources that can assist in the consumers recovery, developing an understanding of the process of recovery and encourages consumers to take action on their own behalf toward recovery.
    5. Exhibits leadership and team-building skills to promote a person-centered, consumer-accountable culture among the team, among the department and across the agency.
    6. Assists in updating, developing and implementing policies and procedures in compliance with facility policies and the Mental Health Rehabilitation policies.
    7. Assists Clinical Manager in auditing client's records to ensure compliance with quality standards and specifically monitors treatment plans, progress notes, medication, education and management, discharge summaries and plans.
    8. Ensures compliance with regulations and guidelines issued by the District of Columbia Department of Behavioral Health and Medicaid Program and other applicable laws and regulations and professional standards.
    9. Provides crisis intervention as needed, refers consumer to other services/agencies for needs (i.e. linking consumer to primary health care physician), Completion of Diagnostic Assessment updates, Treatment planning/Plan of Care updates, Individual, Family and Group Therapy, Request for Insurance Authorization additional units, Documentation in encounter notes, Familiar with EMS (Credible/ Data Wits). On call crisis phone scheduled is maintained by Director.
    10. Completion of referrals to ACT, SUD or other internal programs. Completing discharge planning and/or transfers of consumers. Provides assistance with consumer support recovery and psychoeducation.
    1. Must provide minimum of 8 hours weekly of counseling to outpatient consumers, or through other billable services.
    2. Familiar with DSM-IV diagnostic criteria. Shall ensure to comply with all Federal and District laws and regulations pertaining to scope of practice, licensing requirements and supervision requirements.
    3. Provides treatment planning, plan of care implementation activities that are separate from the diagnostic assessment service.
    4. Maintains client records and completes all documentation on a "real time" basis. Must document an encounter note that indicates the intended purpose of that service, action taken, and results achieved.

    WORK ENVIRONMENT

    This job varies from a professional office environment to field/community. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

    PHYSICAL DEMANDS

    Position does involve local travel on a consistent basis.

    POSITION TYPE AND EXPECTED HOURS OF WORK

    This is a full-time hybrid position. Days and hours of work are Monday through Friday, 8:30 a.m. to 5 p.m. On call hours

    TRAVEL

    The position involves local travel on a limited basis.

    REQUIRED EDUCATION AND EXPERIENCE

    1. Must have at least at least 2 years of work experience in servicing persons with mental illnesses, dual diagnosis/comorbidity with physical health disorders, substance use disorder diagnosis, crisis management, intensive case management, individual and/or family therapy, skill training, and other rehabilitation supports preferred for adults.
    2. Masters' Degree or higher required
    3. Licensed in the District of Columbia: Physicians, Psychologists; LICSWs, LGSWs, APRNs, RNs, LISW, LPCs, LMFTs, and LGPCs.
