    Description

    Job Title: Care Coordinator at Home Care Evolution

    We are looking for a passionate Care Coordinator to be part of our team at Home Care Evolution. In this role, you will collaborate with patients to assess their medical needs, create personalized care plans, and ensure top-quality healthcare.

    Responsibilities:

    • Work closely with healthcare professionals, patients, and families
    • Review and update care plans as needed
    • Accompany clients to medical appointments
    • Set goals and track progress
    • Recruit, train, and manage staff
    • Handle a client caseload of 30-40 individuals

    Qualifications:

    • Prior experience in care coordination preferred
    • Medical assistant certification is a plus
    • Strong problem-solving and organizational abilities
    • Ability to multitask and prioritize effectively
    • Excellent communication and interpersonal skills
