- The LPN Care Manager is a DRIVING position and will require travel throughout Stark, Tuscarawas, and Columbiana counties.
- This is a Full-Time position, M-F 8:00am-4:30pm. No weekends/Holiday Rotation
- We provide superior care and superior services to patients at their end-of-life journey. Only those who have a heart for hospice will succeed.
- Active LPN license in the State of Ohio without any board actions.
- Two years acute care nursing experience preferred.
- CPR Certified
- Computer skills sufficient to properly document services and care.
- Ability to drive during daytime, nighttime, or inclement weather.
- Valid Driver's License with Safe Driving Record
