    Th Program Manager will oversee program operations of the Community to Home program (C2H).

    The C2H program will develop collaborative relationships with mental, behavioral and physical health services as well as social services organizations.

    The C2H will provide home-visiting services to families residing in the south-central rural counties of Pennsylvania to support the needs of children with special healthcare needs ages 0-21 and their families.

    The Program Manager will be required to interface with medical practices, pediatric clinic staff, managed care organizations, service providers, intermediate units and families frequently, as well as supervise C2H Care Managers/Community Health Workers.

    Manager will be working remotely and expected to cover work across 16 counties:

    Northeast counties:

    Carbon, Columbia, Monroe, Pike, Schuylkill, Susquehanna, Wayne and Wyoming South Central counties:

    Adams, Blair, Franklin, Fulton, Huntingdon, Juniata, Mifflin and Perry.

    Responsibilities:
    Provide daily management of the program activities.

    Works with the Assistant Director and Family Services programs staff to assure the contractual-grant-funded projects are implemented properly and timely program deliverables are met.

    Establish and maintain strong relationships with funders, providers, and project staff to ensure effective communication and efficient program implementation.
    Initiate and maintain connections with pediatric providers and other potential referral sources throughout south-central and northeast PA.

    Collaborates with other C2H vendors and the Department of Health Program Officer to assess and determine most appropriate practices and resources across the State.

    Works closely with C2H staff to build capacity that meet the needs of consumers and supporting community health worker training.

    Coordinates and/or conducts all initial intakes with newly referred families, including assessing both child and family needs, over the phone or in-person as needed.

    Reviews and ensures accurate and timely program paperwork submission by staff before completing data entry.
    Works with Assistant Director to assure goals and deadlines are met.
    Assists with evaluations of the effectiveness of service.
    Completes program reports and other written documentation.
    Maintains accurate records according to grant funding and observes HIPAA requirements; generates data and reports appropriately.
    Attends the required meetings regularly by phone or in-person.

    Keeps abreast of up-to-date events and services available for children with special healthcare needs in Pennsylvania and shares information and resources with staff as appropriate.

    Reviews and integrates latest ideas and concepts with Assistant Director in order to improve project delivery, content, and/or evaluation for target audience.

    Participates and initiates conversations that bring forth innovative ideas and concepts that align with Health Promotion Council's (HPC's) strategic plan in regularly scheduled team meetings.

    Participates in grant writing tasks as needed by upper management level to develop concepts, write proposals to grow organizational initiatives; contributes content expertise for grant proposals as needed.

    Ensures educational materials are culturally appropriate for diverse populations, especially for individuals with limited reading and English skills.
    Participates in (HPC supervisor team meetings regularly.
    Represents Community To Home (C2H) and HPC at various meetings.

    Establishes and maintains linkages with medical providers, community health, social service, education and legal service agencies, and other support services through effective communication between C2H staff, community members, and C2H partners, including pediatric practices.

    Conduct regular reflective supervision of C2H staff, including case review and oversight of client progress.
    Serve as the primary point of contact for medical providers and pediatric staff serving C2H - enrolled children.

    Assist staff with the use of program materials, written forms, assessments, case notes, referral forms, discharge plans, and other written documentation, and entry of these forms into the online database.

    Support small caseload of the most intensively needy families alongside (C2H staff.
    Support staff in defining priorities, deadlines, opportunities for growth, and resolving any program challenges.
    Lead program hiring and oversees training, particularly community health worker training, and orientation of new program staff.
    Contributes to publicizing program/ program outcomes, including abstracts, posters, and presentations.
    Mentors staff and fosters professional and personal growth.
    Participates in case conferences, trainings, case reviews, staff meetings, community fairs, etc., as required.
    Writes and conducts annual staff performance management reviews in a timely manner.
    Communicates formally on a weekly basis with Assistant Director.
    Enforce agency policies, procedures and other relevant duties as assigned.

    Advocacy on behalf of r clients for community services and to address unmet needs.
    Excellent problem solving, conflict resolution, and professional communication (written and oral) skills.
    Ability to establish priorities, work both independently and in a team environment to meet objectives with minimal supervision.
    Advanced proficiency in Microsoft office suite and various web-based platforms with the ability to learn new software, as needed.

    Experience and Job Requirements:
    Master's degree in social work or closely relevant fields.
    At least four years of demonstrated ability to effectively supervise and lead staff.

    Demonstrated experience working with health care providers, and knowledge of issues related to maternal and child health, child development, child abuse prevention, and children with special health care needs.

    Experience working with diverse populations and low-income individuals.
    Must clear child abuse, criminal history check, and FBI clearance
    Ability to travel throughout Northeast and South-Central Pennsylvania.
    Ability to acquire information about new technological systems, organizations and practices.
    Experienced in working with " motivational interviewing "
    Experience in data collection/entry, evaluation monitoring and needs assessments.
    Preferred
    Home visiting experience.
    Experience interacting with medical providers and social service agencies and resources in Northeast and/or South-Central region of Pennsylvania.
    Knowledge of public health theories, principles, and practices.
    Experience monitoring budgets.
    Experience with grant writing for public health or related field.
