    Patient Care Manager

    Responsibilities:
    • The Patient Care Manager will oversee the delivery of patient/family services, ensuring compliance with regulations
    • Supervise direct patient care services and interdisciplinary team components
    • Manage and evaluate RN Case Managers, Social Workers, Chaplains, and other staff

    Qualifications:
    • Registered Nurse with state licensure
    • Minimum of 3 years' experience in hospice, home health, or oncology
    • Supervisory experience preferred
    • Expertise in crisis intervention, pain management, and family counseling
    • Strong administrative and communication skills

    Duties:
    • Ensure nursing services adhere to hospice program policies
    • Oversee availability of nursing services round the clock
    • Lead orientation and training programs for nursing staff
    • Act as a liaison with external agencies
    • Participate in quality improvement initiatives
    • Supervise patient/family care delivery
    • Conduct interdisciplinary team meetings and staff support sessions
    • Manage medical records and compliance
    • Provide evaluations for team members
    • Promote resource utilization for team and patients
    • Identify staff education needs and assist in program development
    • Assist in volunteer and bereavement programs
    • Participate in staff selection and orientation
    • Manage budget for clinical components
    • Accept additional assignments as needed
