    Job Description

    Summary of position:

    The Patient Care Coordinator (PCC)/Supervisor of Nurses (SON diagnosis and treats human responses to actual or potential health problems through such services as data collection, physical assessment ,case-finding, health teaching, health counseling and provision of care supportive to /or restorative of life and well-being as data collection, physical assessment, health teaching, health counseling, and provision of care supportive to or restorative of life and well-being and executing medical regimens prescribed by a licensed physician , dentist or other health care provider legally authorized under this title and in accordance with the commissioner's regulation. Under the guidance of the On-Site Director of Nursing (OS-DON), the Patient Care Coordinator/Supervisor of Nurses is responsible for directing and administering nursing care to patients. The/he/she coordinates activities through collaboration with physicians, administration, pharmacy, clerical, and correctional staff, etc. They/he/she is guided by a thorough knowledge of principles of nursing based on biological, physical, and psychosocial sciences. The/he/she possesses a working knowledge of principles of leadership and utilizes that knowledge in guiding and directing nursing staff. They/he/she assists the OS- DON in ensuring compliance with standards, policies

    and procedures of NYC Health and Hospital Corporation Correctional Health Services contracted with Prison Health Services, Inc. and the Department of Nursing. They/he/she will be working with adult male and females between sixteen to eighty and infants from birth to one year of age.

    Responsibilities include:

    1. Provides direct patient care as necessary. Using the problem-oriented progress notes, written to match each problem documented on the Problem List and organized using the SOAP format.
    2. Coordinates the nursing care provided for patients within a given unit by:
    3. Assigning appropriate nursing staff for patient care activities such as admissions, sick call, treatments, medications, punitive segregation rounds, follow-up, etc. Problems in staffing must be communicated to the On-Site Director of Nursing.
    4. Utilize therapeutic communication skills and effective interviewing techniques to elicit pertinent historical information and to make appropriate referrals as required. Screen patients with mental health problems in an immediate manner and makes appropriate referral as required.
    5. Making rounds to patient care areas for the purposes of assessing, planning, implementing, documenting, and evaluating unit nursing care delivery. Assist OS-DON in staffing needs. Provides guidance and assistance to nursing personnel in the delivery of patient care.
    6. Assuring continuity of care on other shifts through written and/or verbal communication. Communicates special care needs of particular patients to the psychiatrist, On-Site Director of Nursing or the On-Island Administrator.
    7. Collaborates with the OS-DON in the orientation of new staff on the unit. Evaluates the new nurse on an ongoing basis; writes a formal evaluation at the end of the probation period, and makes recommendations to the OS-DON.
    8. Evaluates and provides feedback to staff on an ongoing basis and in an annual formal evaluation conference. Collaborates with the OS-DON and/or Deputy Director of Nursing. Provides appropriate documentation and records of nursing personnel activities.
    9. Identifies staff development needs and plans strategies to meet the needs in collaboration with available nursing resources.
    10. Conducts meetings with his/her staff, in collaboration with the On-Site Director of Nurses, to share information, discuss problems and potential solutions, and to promote cooperative professional relationships among the staff.
    11. Monitors the quality of patient care through quality assessment -improvements mechanisms.
    12. Communicates, documents and collaborates on an ongoing basis, with the On-Site Director of Nursing concerning patient care and unit administration.

    COORDINATOR / SUPERVISOR RESPONSIBILITES:

    1. Coordinates the intra-disciplinary team providing comprehensive patient care.
    2. Familiarizes self with and participates in defining, maintaining and Interpreting standards of nursing practice through the application of nursing policies and procedures according to the Prison Health Services, Inc. Policy and Procedure Manual and specific policies of the Correctional Health Services.
    3. Identifies those components of the nursing plan, which may be delegated to allied nursing and other non-professional personnel, and provides teaching, guidance and supervision as necessary.
    4. Assists with the unit orientation of nursing personnel through participation in the precepting function.
    5. Counsels nursing staff and directs information to the On-Site Director Of Nursing. Provides documentation as needed on a timely basis.
    6. Supervise all care provided by LPN's/NAs and assumes other related responsibilities as required.
    7. Administer medication in a safe and efficient manner consistent with

    prescribed orders, mouth checks, and established standards.

    1. Participates in a system of ongoing monitoring activities, including peer, content, and quality reviews to ensure delivery of quality care. Makes recommendations for change where problems are identified.

    3. Participation in the evaluation process.
    4. Formal undergraduate, graduate and post-graduate courses.
    5. Attendance at continuing education programs sponsored by community and

    other health agencies.

    1. Participation in unit conferences and staff development programs.
    2. Participates in projects and research activities that identify patient care needs.
    3. Shares with peers the knowledge that is gained at continuing education Programs

    CARE OF THE ENVIRONMENT

    1. Contributes to an environment that is conducive to meeting patients' needs

    by taking initiative in maintenance of supplies and equipment.

    1. Demonstrates awareness of the correctional health environment through

    observance of security regulations and appropriate care of contraband.

    1. Contributes to maintenance and/or improvement of the unit morale by:
    2. Utilizing established channels of communication.
    3. Recognizing, accepting, and respecting people as individuals.
    4. Demonstrating concerns for unit and patient needs over a 24 hour period.

    Department Preferences:

    • Demonstrate commitment to continuing professional development through attendance at continued education programs and participation in a professional /organization committees/councils.
    • Has three years of current professional nursing experience in a correctional setting or three (3) years of professional psychiatric nursing experience in an ambulatory/ hospital, clinic, alcohol/drug rehabilitation unit or nursing convalescent home, crisis intervention experience is preferred.
    • Demonstrated clinical competence and leadership traits (e.g. decision-making ability and skills in problem-solving, communication, priority-setting, and interpersonal relationships)

    Qualification Requirements:

    • Licensed as a registered professional nurse in accordance with State Statures or eligible to practice nursing in accordance the State's Board of Nursing.
    • Current Basic Cardiac Life Support certification required - ACLS preferred

    Minimum Qualifications

    For individuals holding an unrestricted registered professional nursing license BEFORE July 1, 2020 in New York State, another state or United States territory:

    1. A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and
    2. Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support (BLS) through the American Heart Association (AHA); and
    3. Three (3) years of experience as a registered nurse

    For individuals issued a registered professional nursing license on or AFTER July 1, 2020, based on expected implementation of Chapter 502 of the Laws of 2017 and Chapter 380 of the Laws of 2018:

    1. A valid New York State license and current registration to practice as a Registered Professional
    2. Nurse in New York State; and
    3. A Bachelor's of Science degree in Nursing or higher (or other nursing degree, as identified by NYS Education Department including Master of Science in Nursing, Doctor of Nursing Practice, Doctor of Philosophy Degree in Nursing or Doctor of Nursing Science) from an accredited college or university, registered with the NYSED; or
    4. Obtains a Bachelor's of Science Degree in Nursing or higher, as described in "a" above, within ten
    5. (10) years of the date of initial licensure; and
    6. Holds, or obtains through facility orientation, a valid and current certification in Basic Life
    7. Support (BLS) through the American Heart Association (AHA); and
    8. Three (3) years of experience as a registered nurse
