- Conduct comprehensive client assessments and create housing goals
- Screen, place, and provide case management for individuals in the 'Safe Park' program
- Enhance clients' quality of life and move them out of poverty
- Assess and verify client needs for financial assistance
- Provide case management services including goal setting and progress monitoring
- Maintain a caseload of 50 families per year
- Connect clients with community resources and follow up on referrals
- Coordinate with service providers and input data accurately
- Provide required documentation and coordinate health-related events
- Participate in staff meetings, training programs, and adhere to agency policies
- Qualifications:
- Bachelor's Degree in Social Work or related field
- Fluent in Spanish
- Minimum of 2 years nonprofit sector experience
- Previous experience with family case management
- Knowledge of community resources and social issues
