- Welcome youth to the program, ensure they feel comfortable, safe and understand the program structure, opportunities for engagement and access to supplies that they might need;
- Review intake documentation and ensure it is entered into ETO and HMIS upon intake.
- Review and integrate the Initial Background Interview (IBI) into the Individual Service Plan and the accessing of resources for the youth.
- Develop an Individual Service Plan (ISP) with the youth upon completion of assessment.
- Revise and update the ISP as necessary. An updated plan is written, at minimum, after two weeks of intake, and upon the transition to a new Phase in the program.
- Maintain a calendar of the youth's work schedule, appointments, etc., in conjunction with the case plan.
- Develop and maintain knowledge of and connection to community resources.
- Make appropriate referrals for services within CHC and to community agencies.
- Engage in positive transition planning with the youth.
- Document regular progress notes in youth's case record (HMIS and ETO) for those youth assigned to his/her case load.
- Complete documentation specific to external grants as required;
- Document pertinent information in Transition Log, Critical Incident Reports, Follow-Up forms, etc., as necessary.
- Prepare and present assigned cases at weekly case review meetings and monthly Client Progress Review Meetings.
- Facilitate psycho-educational and support group activity with youth on a regular basis.
- Provide custodial supervision as follows:
- facilitating group case management meeting with youth;
- ensuring the cleanliness and safety of the facility;
- providing crisis intervention, as necessary;
- providing regular supervision of residents and activities.

Covenant House CA (CHC) is excited to announce the launch of its new transitional housing site in Santa Clara, CA, for youth experiencing homelessness. Made possible by a unique public/private partnership, Covenant House opened the site in June 2021, marking CHC's fifth site in CA. The staff at CHC Santa Clara will be part of a unique experience of being the first staff to ever work at the site, not only becoming a critical part of CHC's history, but helping to develop a program that most effectively meets the needs of youth experiencing homelessness in the Santa Clara Valley. The staff will draw upon CHC's 30+ years of experience serving youth, ages 18-24, while developing unique programming and services to meet the unique needs of youth in the SC Valley. CHC is committed to meeting young people where they are, accepting them for who they are, and relentlessly engaging them so that they can fulfill their dreams. POSITION SUMMARY: Under the supervision of a Director, the Case Manager is responsible for the provision of case management and custodial services within the CHC transitional housing program. Essential functions of the position are youth intake and assessment, case planning, documentation in multiple databases, and workshops including community building and support. The position is also responsible for providing these services within the framework of our Mission Statement, our Code of Conduct, and the Policies and Procedures of Covenant House California. As an hourly, non-exempt position, meal periods are required within the first five hours of the workday and are non-paid, as well as two 10-minute breaks are required to be taken each shift. Any overtime must be pre-approved by a supervisor.
CLASSIFICATION: Full-time non-exempt 40-hours per week
SALARY RANGE: $25.00 per hour
ESSENTIAL FUNCTIONS:

The goal at Covenant House California is to develop positive relationships with the residents through the effective utilization of the case management system. This system includes:

JOB REQUIREMENTS:
EDUCATION: Bachelor's degree in Psychology, Sociology, Social Work or related field OR an Associate degree in Psychology, Sociology, Social Work or related field and 2 years of case management experience, preferably with homeless or runaway youth.
EXPERIENCE: In lieu of a bachelor's degree, must have a minimum of 4 years of case management experience, preferably with homeless or runaway youth. Must be fluent in Spanish.
SKILLS: Must be a self-starter and team player with the ability to work with other staff. Knowledge of adolescent development, community resources, and case management skills. Must have good organizational skills and the ability to communicate with members, clients, vendors, and the public. Communication with and about youth must reflect a strength-based and trauma-informed approach. Bilingual in Spanish is preferred but not required.
TRANSPORTATION: Must have a valid CA Driver's License, reliable transportation, and a good driving record.
WORKING CONDITIONS: General business/office environment. Must be able to lift up to 40 lbs. on occasion, bend, sit and stand for prolonged periods of time. Some weekend/holiday work may be required. Full-time 40 hours per week.
SUPERVISORY DUTIES: This position does not perform supervisory duties.
EQUIPMENT USED: Must be able to utilize basic word processing, email, and internet programs. Must be able to communicate effective both orally and in written form.
