Position: Clinical Case Manager

PATH is looking for compassionate individuals to join the team as a Clinical Case Manager at the Villas on the Park PATH San Jose office location.

About the Company

Since its inception, PATH (People Assisting The Homeless) has been at the forefront of innovative approaches to support individuals experiencing homelessness. Operating across California, PATH offers a wide range of services aimed at preventing homelessness, providing street outreach, employment assistance, case management, supportive services, transitional housing, and permanent supportive housing.

About the Job

As a Clinical Case Manager, you will work in collaboration with a team to deliver direct client outreach, linkage, and retention services to individuals facing homelessness. The goal is to facilitate recovery, community integration, and ultimately break the cycle of homelessness.

Position Responsibilities Include:

Conduct comprehensive assessments and develop intervention plans
Create individualized treatment plans
Offer ongoing case management support
Provide field-based therapeutic services
Collaborate on crisis intervention and risk assessment
Ensure crisis intervention services and coverage for the After Hours Response phone line

Preferred Qualifications

Licensure or registration with the California Board of Behavioral Sciences

Minimum Qualifications

Master's Degree in Social Work or equivalent
Experience in healthcare, case management, or working with vulnerable populations

Mandatory Requirements

Employment eligibility
CPR/First Aid training
Passing Tuberculosis Test, Background Screening, and Drug Test

Driving Requirements

Driving is a key aspect of this role. Candidates must have reliable transportation, a valid driver's license, and insurance coverage for personal vehicles used for work.
