As the Targeted Case Manager, your work WILL make an impact every single day.

Your attention to detail and collaboration in a team environment will help our clinics run smoothly, ensuring patients get the individualized care they need.

The Targeted Case Manager (TCM) provides support to patients by working with the treatment team and other recovery support staff to identify needs and facilitate connections to resources, supports, and services available in the community.

The TCM will make referrals to appropriate community resources to assist patients in dealing with the barriers they face in areas such as housing stability, legal challenges, access to transportation, physical health needs, economic stability, community integration and access to social support, leisure, and recreation, etc.

They will maintain close communication with all involved parties for minimal interruptions in care.

Also, they will work with patients to identify their own individualized recovery goals and resource connections, ensuring patients are receiving holistic support and that all parties involved in patient care are cohesively collaborating.

Bachelor's degree in Behavioral Health Sciences.
At least one year of direct work experience is required.
