    Description

    The Care Team Manager:


    Effectively manages a team of clinical and support colleagues working in an in-patient environment, maintaining accountability for providing the highest quality of clinical care and psychosocial support for patients and families.

    Provides positive leadership of a clinical team composed of a variety of disciplinary practices to ensure patient care efficiency, effectiveness and satisfaction.

    Promotes continued professional development.

    Serves as clinical team expert and manages high risk and complex patients/family/staff situations, accessing available resources, maintaining effective communication and optimizing quality patient care.

    Collaborates and communicates effectively with associate medical director to initiate appropriate level of care on admission and changes in level of care.

    Co-facilitates Interdisciplinary Group meeting.

    Performs ongoing review of patient documentation to ensure the documentation is complete, accurate and timely and to verify that the electronic medical record system (EMR) is being utilized appropriately.

    Maintains responsibility for bed management activity and monitoring inpatient length of stay. Monitors for staffing and census fluctuations and provides direct patient care as needed.

    Ensures compliance with all accrediting agencies, State, Federal, and local government regulations; maintains a strong position against fraud and abuse.


    Educational/Professional:
    Must possess current and active Florida licensure as RN; BSN preferred. CHPN a plus.
    Minimum of 2 years recent community-based hospice experience required.
    Recent acute level clinical experience required; GIP care experience preferred.
    Previous management experience required; healthcare environment preferred.
    An equivalent combination of professional experience and education may be considered as meeting the educational/professional requirements.
