    The Long Term Services and Supports (LTSS) area of Human Services Department is searching for two Case Management Assistants (CMA) to join the team. The CMAs will work alongside Public Health Nurses and Social Workers, focusing on post-assessment tasks related to accessing programs and services for individuals in need. The team administers Medicaid waiver and state programs for various vulnerable groups. The roles take a person-centered approach to their work.

    Location and Hours:

    - The positions are remote, with occasional on-site requirements for meetings and trainings. Work hours are Monday to Friday, 8 a.m. to 4:30 p.m. Candidates must reside in or relocate to Minnesota.

    - New remote/hybrid hires between January 2, 2022, to December 31, 2024, will receive support for internet connectivity costs after 6 months of employment.

    About the Positions:

    - Full-time positions with benefits as Case Management Assistants.

    In these roles, you will:

    • Follow up with clients and assist in completing required county and state documents.
    • Review files and manage client information from various databases, including Microsoft Excel.
    • Maintain client records, track demographic and service data, and handle ongoing case updates.
    • Collaborate with external partners, manage data for reports, and ensure compliance with procedures.

    Requirements:

    • Social service experience, human services certificate, or relevant college coursework.

      • Nice to Have:

      • Experience with databases and programs, working with diverse populations, and proficiency in Microsoft Office Suite.
      • Ability to communicate effectively, make independent judgments, and work well within a team.
