To improve access to services, develop responsive programs, listen to the voices of people with disabilities, chronic conditions, and older adults and honor the desire for independence and self-determination. CLA's current services and supports include personal care, chore services, behavioral health, and service design and coordination.

    CLA is committed to person centered service design and delivering supports that reduce the everyday challenges faced by individuals who choose to partner with CLA and their families.

    • Full Time or Part Time Positions
      • FT - Mon-Fri, 8:30AM-4:30PM, 37.5 Hours Per Week, NO WEEKENDS
      • PT - Mon-Fri, 20-29 Hours Per Week, NO WEEKENDS
    Salary/Wage Range:

    Hourly $21.00 to $21.00

    Position Summary:

    The Community Support Specialist (CSS) works with Community Living Alliance (CLA) clients' to facilitate mental health and/or AODA recovery and support them to achieve their highest level of independent functioning, stability, and community connection. As part of their work CSSs' conduct client assessments and care plans, coordinate and facilitate services, and support continuity of care upon discharge.

    • Supervision available for those actively accruing hours for LCSW, LPC, LMFT or other related professional certification.

    Primary Duties:
    • Provide support for individuals working toward recovery using a supportive, strengths-based, and culturally humble approach to treatment.
    • Facilitate and coordinate services relevant to individuals' recovery plans and community support needs.
    • Utilize effective strategies to engage program participants, enhance motivation, and encourage meaningful, collaborative, and strength's-based goal-planning and progress.
    • Provide screening and assessments, service linkage, service facilitation and other allowable and appropriate services on the CCS service array in a manner that is trauma-informed, culturally relevant and non-stigmatizing.
    • Assist the program participant with identifying and establishing natural and professional supports in the community.
    • Maintain contact with program participants to a degree sufficient to support effective engagement as well as monitor and assist progress.

    Qualifications:

    Minimum Education/Experience and Requirements:
    • Six (6) months experience providing psychosocial rehabilitation services to individuals with mental health and/or substance abuse diagnosis, required.
    • Six (6) months experience working with individuals with mental health and/or substance use conditions, intellectual or physical disabilities, chronic health conditions or seniors, required.
    • Case management experience, community based mental health services and/or working with people with disabilities, preferred.
    • Comprehensive Community Services (CCS) credential in Dane County WI, preferred
    • Valid driver's license, reliable transportation, and current auto insurance to serve clients throughout Dane County
    • Must be at least 18 years of age or older.
    • Must be Authorized to work in the United States.
    Physical Demands

    Capable of lifting/moving/pushing/pulling 35 pounds
