    Senior Director of Health Services


    Summary:


    The Senior Director of Health Services provides clinical and administrative leadership to the Primary Care, Oral Health, Psychiatric Services and support services at Heartland Alliance Health (HAH).

    In this role, the Senior Director of Health Services is responsible for the quality of integrated health care provided to participants, emphasizing the prevention and treatment of disease, and for the clinical productivity of the provider staff.

    It is primarily an administrative role and on occasion, this individual may provision direct patient care.

    The Senior Director of Health Services provides thought-leadership to the organization while leveraging industry knowledge and best practices to promote the mission and vision of Heartland Alliance Health.

    Supports culture development and management efforts.


    Essential Functions:
    Duties & Responsibilities

    Strategic Leadership


    • Senior Director of Health Services ensures coordinated, continuous, and culturally competent care is provided to patients.
    • Works collaboratively with external professionals and community agencies, to provide high quality care, and the health center and outreach lead providers, Oral Health Director, Psych Director, and Nurse and MA managers who will be responsible for managing the clinical activities on a day-to-day basis.
    • Ensures compliance with HRSA and FTCA requirements
    • Meets regularly with providers to review care standards and performance.
    • Identifies and creates opportunities for community and professional outreach to maintain and increase patient referral base such as providing educational presentations, attend health fairs.
    • Participates in quality improvement activities to develop medical and joint practice protocols and identify appropriate evaluative criteria for utilization review activities, evaluate care based on clinical standards through the quality improvement process.
    • Adheres to professional standards as outlined by governmental bodies, American Medical Association (AMA), American College of Obstetrics and Gynecology (ACOG), American College of Physicians, American Academy of Pediatrics, and American Academy of Family Physicians, HRSA, private funding sources, organization plans/policies and unit guidelines.
    • Interface with Director of Health Center Operations to ensure efficient and effective health center operations.
    • Collaborate with Senior Director of Quality Management and Practice Improvement on quality and risk management issues including patient safety and delivery of quality care through peer reviews etc
    Operations and Metric Performance Management


    • Partners with leadership to ensure completion of activities not limited to approving time cards, time off request, and coordination of peer reviews, efforts to meet productivity objectives and improve quality.
    • Provides practice oversight for health care operations in health center facilities, community and outreach settings.
    • Measures and monitors clinical productivity of all assigned staff, develops productivity standards, and reports to the senior leadership team regularly on performance relative to standards.
    • Oversees the Integrated Health Care team in program planning, monitoring and evaluation of health care services including implementation of Patient Centered Medical Home.
    • Adheres to collaborative practice principles of respect and joint decision making when collaborating with all stakeholders.
    • Participates in appropriate educational activities in order to maintain Board certification and continue professional growth.
    • Documents comprehensive, accurate and continual data on clinical care and reports according to organization standards.
    • Prepares the appropriate program reports to fulfill funding requirements as assigned.
    Departmental Management & Supervision


    • Measures and monitors quality of patient care services in Primary Care, Dental Care, and Psychiatric Services, as offered by HAH providers, including Physicians, Advanced Practice Nurses, Physician's Assistant, Registered Nurses, Medical Assistants, Psychiatrists, Dentists, as well as, the care provided by ancillary and medical support staff.
    • Organizes and facilitates through collaboration with HAH clinical staff development activities such as clinical practice updates, journal clubs and chronic illness collaborative to ensure that education and training are provided.
    • Works with the EHR Clinical Application Support Specialist and the Director of Data Analytics or other peer relationships to ensure all participant charts are updated and in compliance with organizational standards.

    Supervisory Responsibilities (if applicable):

    • Senior Director of Health Services supervises the Health Center and Outreach Lead Providers, Director of Oral Health, Psychiatric Director, Nurse Manager, and Manager of Medical Assistants. Other Primary Care Services staff may be supervised by the Senior Director Health Services, as assigned

    Education and Experience:
    * · Candidates must hold either a Doctor of Medicine, Doctor of Osteopathy, Physicians Associate or an Advanced Practice Registered Nurse (APRN) degree.
    * · Have three years of clinical experience, one year of administrative or supervisory experience, preferably in an ambulatory setting. Documented experience in protocol development, evaluation and implementation of clinical standards of care based on the quality improvement process. Experience in establishing and maintaining effective working relationships with other employees, patients with the public.


    Certifications/Licenses:

    • Doctor of Medicine or Doctor of Osteopathy: Graduation from a college or university accredited by the American Medical Association with a Doctor of Medicine degree or accredited by the American Osteopathic Association with a Doctor of Osteopathy degree. If medical education occurred at a foreign medical school, the applicant must possess a current Educational Council Foreign Medical Graduate (ECFMG) certificate; or
    • Physician Associate ( PA) : Graduation from an accredited Physician's Associate program and a national PA certification; or
    • Advanced Practice Registered Nurse( APRN): Graduation from an accredited APRN programs and a national APRN certification.

    Knowledge/Skills/Abilities (K/S/A):

    • Skill in developing staff by providing supervision, oversight and training, while effectively delegating responsibility, and collaborating with staff to organize their objectives.
    • Perform effectively under the stress of frequent interruptions and/or distractions.
    • Ability to establish and promote cooperative working relationships to integrate staff to provide high level of patient care.
    • Ability to interact and communicate with all levels of staff and customers, internal and external, to serve as liaison between clinic administration, medical staff, and customers maintaining professional demeanor at all time.
    • Previous experience in a federally qualified health center, working with HRSA grants, with Patient Centered Medical Home requirements and accreditation process, working in a setting with integrated dental, medical and behavioral health services, experience managing in a union environment, experience in clinical quality improvement .
    • Access to regular, dependable and timely transportation
    • Ability and comfortability working with people of varying socioeconomic backgrounds.
    • Excellent communication skills. Strong networker, convener, relationship builder and communicator with experience collaborating with diverse work teams and stakeholders.
    • Demonstrated ability to handle confidential and sensitive information possessing skills to handle situations in a professional and diplomatic manner.
    • A high degree of collaboration and emotional intelligence, self-motivation, and analytical ability.
    • Proficiency in Microsoft Office, Internet Explorer, Outlook/Exchange; Windows operating systems; and other software routinely used by Heartland Alliance.
    • Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.
    • Ability to work independently and remain self-motivated with minimal direct supervision.
    • Strategic thought partnership and consultancy skills.

    Work Arrangement:
    This role is expected to operate in an in-person setting.

    • Each program site operates on a 24/7 basis, 365 days per year. To meet both the needs of our participants and contractual obligations, employees are required to be flexible in regards to scheduling and work location.
    • Program sites are located throughout the Chicago area, including the North and South sides of Chicago, as well as the suburbs. Employees may be required to report to any one of our locations on a temporary or permanent basis.
