    HOSPICE COMMUNITY SPECIALIST -GWINNETT COUNTIES

    Tasked with spearheading the formulation and implementation of territory and account strategies aimed at sustaining and expanding hospice service admissions from referral partners across the Branch service area.

    These referral partners, encompass entities such as physician offices, senior living facilities, rehabilitation centers, skilled nursing facilities, and select hospitals.

    Oversight extends to approximately 30-50 accounts on a rotating bi-weekly call schedule, with identified accounts incorporated into the call cycle.

    While the primary liaison?s role is with the referral source, occasional interaction with patients and families may be necessary.

    Responsibilities entail explaining hospice care to families and patients, collaborating with referral partners, the Hospice Atlanta nursing team, and facility personnel, as well as assisting in administrative tasks.

    The ability to collaborate within a team, adapt to dynamic business strategies, and employ analytical acumen to assess opportunities leveraging territory insights and organizational resources is integral to thriving in this role.
