Description
The Stroke Pain Coordinator is responsible for managing the efforts of the hospital to ensure evidence based care is being provided to quality measure patients.
Performs concurrent chart review and works closely with Medical Staff, Nursing, and Case Management to identify patients who are part of the Quality Indicators diagnosis groups (AMI, COPD, PN, STK, I-IF, Surgical Care, Sepsis, and WE).
A key role of this position is professional ongoing follow-up to ensure best practices are being met for each patient, which will also include periodic Quality Analyst IRR Chart Reviews.
As Stroke Program Coordinator, ensures ongoing compliance with The Joint Commission Stroke Regulatory Standards, Education requirements, and stroke population chart abstractions, with data entry into the American Heart Association Get With the Guidelines Registry.
This position is responsible for the aggregation, analysis, and presentation of Stroke data for monthly review and ongoing performance improvement efforts.
QualificationsEducation:
BSN
Experience:
Minimum 3 years' experience in an acute care facility. Demonstrated clear knowledge of TJC Quality Measures and Stroke Certification Standards. Demonstrated computer skill proficiency.
Licenses/Certifications:
Current license as a Florida registered nurse.
